I, JOCELYN L. METRILLO of Barangay Sinabaan, Bautista, Pangasinan having been duly appointed to the
position of Barangay Nutrition Scholar (BNS) do hereby solemnly that I will faithfully discharge to the
best of my ability the duties of my present position and of all others I may hereafter hold under the
Republic of the Philippines that I will support and depend the Constitution and bear all allegiance to the
same that I will obey the law legal orders and decrees promulgated by the duly constituted authorities
of the Republic of the Philippines and I will impose this obligation upon myself voluntary any mental
reservation of purpose of evasion.
