Referred to this Office by the Regional Director of the Department of Education, Regional
Office 10, is the attached letter-complaint for alleged grave misconduct lodged by Ms. Iris Lynn Samson
Impas against Ms. Leoma Ortega Tabanao, public school teacher at Ignacio Tan Memorial Integrated
School, Bonifacio East District, this Division, for fact-finding/preliminary investigation.

In the foregoing regard, a Fact-Finding/Preliminary Investigation Committee is hereby


constituted, which shall be composed of the following:

EUGENE I. MACAHIS, JR. - Chairperson


JOHNNY M. BELOY - Member
SUSAN BACO - Member.

The Chairperson and Members of the Committee are hereby directed to conduct a Fact-
Finding/Preliminary Investigation on the above-mentioned matter pursuant to Section 9 of DepEd Order
No. 49, s. 2006, which shall be commenced not later than five (5) days from receipt hereof and to
submit to the office of the undersigned its Investigation Report with recommendation, together with the
complete records of the case, within three (3) days from its termination.

Further, the Chairperson and Members of the Committee are mandated to require complainant
to comply with Section 5 (par. e, Certification of Statement on Non-Forum Shopping) of DepEd Order
No. 49, s. 2006, which provides that the complaint shall be under oath and include the following and to
submit the deficiencies during the investigation, to wit:

e. Certification of Statement on Non-Forum Shopping.

For immediate and appropriate action.
