PAST MEDICAL HISTORY
There had been no previous procedures performed on the patient. However, she has had ailments
in the past, such as colds and flu, as well as headaches. She would take the required prescription soon
away to avoid any negative consequences. Yes, she has been admitted to the hospital before due to the
stress she believes her body is under. She was bored during her stay in the hospital. She didn't want the
hospital's stench. She is not allergic to any drugs, foods, or other chemicals found in the environment.
FAMILY HISTORY
The patients has nine siblings, she is the youngest. She lives now with her own family.
Because her husband works abroad, one of their family get-togethers is video calls, and when she
has free time, she treats her two girls to a treat. She is the one who makes the majority of the big
choices, and she and her husband are the primary financial supporters. They simply need to have
an open platform to discuss the issues and address them as soon as possible. They are currently
dealing with serious financial issues. problems that they are facing now are financial. Her closest

SLEEP-REST PATTERN
The patient usually sleeps at 9:30 pm or 10 pm. She rated the quality of her sleep at 7 hours. She
immediately falls asleep when she is very tired, and sometimes she has difficulty sleeping when the
surroundings are noisy. Her current health doesn't affect her normal sleep habits. Her favorite pillow can
make her fall asleep.
ACTIVITY AND EXERCISE
A. ACTIVITY OF DAILY LIVING
Normal daily activities for patients include getting up early in the morning, drinking
coffee as her breakfast, cleaning the house, washing her clothes, and taking a bath. Her
current physical health doesn't affect any of these self-care activities.
B. EXERCISE ROUTINE
The activity that gives the patients exercise is cleaning the house with matching dancing. She can do this
most of the time.
C. OCCUPATIONAL ACTIVITIES
She is a manicurist in a salon. Sometimes it can affect her because her body becomes weak and she
cannot do the work.

The patient said that before, when she was admitted, the doctor advised her to eat healthy foods and
get enough sleep. For her, the best way to learn something is to interact with several people, and she is
now in good condition. She has no difficulty expressing herself because she is a very expressive person.

The patient describes herself as a very hard-working mom. She only considers that her strength
and weaknesses are her family. She deeply accepts her appearance. The patient has a kind heart because
she gives food or any amount of money if she encounters PWD's.
ROLES AND RELATIONSHIP PATTERN
The patient's occupation is manicurist. Her major responsibilities are to be patient with her
customers and be the best employee. So far, at work, there have been no major problems, and she feels
blessed to be surrounded by those kinds of people. She can't live without her two daughters because they
give her courage in every path she takes. She is not sociable because most of her free time is spent just
inside her house.
HEALTH PERCEPTION & HEALTH MANAGEMENT
The patient is in good health, but her blood pressure spikes when she is exhausted and overthinks.
On a scale of 1 to 10, she would give herself an 8. Hypertension is her current health issue. Her body
weakens and she feels dizzy most of the time when she is really busy and concentrated on her work,
which has a significant impact on her daily activities. When she had hypertension, one of the things she
did was take herbal medicine. She is afraid of the outcome, therefore she refuses to seek medical counsel.
That's why she worked out to keep her body healthy. She asked her daughter to do BP on her and couldn't
recall when she had her last vaccine.
NUTRITION AND METABOLIC PATTERN
The patients drink only coffee in the morning, sometimes she can't eat lunch, and at dinner she
only eats 1/2 cup of rice and eat a lot of dishes. She usually eats her breakfast at 7 am, lunch at 3 pm or
whenever she feels hungry, and dinner at 7 pm. She only eats snacks when she wants. She doesn't take
any vitamin supplements. She doesn't know what to consider for her diet. Mostly, she usually drinks soft
drinks over water. She had no difficulty in chewing or swallowing food and even experienced sore
throats, sore tongues, sore gums, nausea and vomiting, or abdominal pains. She doesn't use any antacids.
BLADDER ELIMINATION METHOD
The patients' urine patterns are not consistent. She urinated two or three times per day, depending
on how much water she drank. Her urine was yellowish in color and she couldn't determine how much of
it she had. She doesn't have any issues urinating unless she has a UTI. She did not have bladder surgery or
a urinary catheterization.
BOWEL ELIMINATION PATTERN
Her bowel pattern is every other day, and it is brown in hue, according to the patient. She claims she
never uses laxatives, enemas, or suppositories. Her bowel movements are fine, according to the patient.
COPING AND STRESS TOLERANCE PATTER
The patient says that dealing with money difficulties is the most stressful scenario in her life. It
causes her anxiety every time she thinks about it. The one thing that helped her cope was going to a
relaxing location. Her stress reliever is singing and jamming with her kids. She frequently turns to her
girls since she finds it easier to express herself with them. She approaches life's challenges by taking
action as soon as possible because, according to her, once you've recognized your problems and set some
constructive goals to address them, you must take action to achieve your objectives.

days long. During her menstruation, she had dysmenorrhea flare-ups.

two daughters, 19 and 20 years old.
VALUE AND BELIEF PATTERN
Her health and her family are the most essential aspects of her existence. Her only hope for
achieving success in life is to work hard and be faithful. God is her main source of hope and strength in
life, because she believes that through prayer, you may speak out about your issues without being judged.
For her, prayer is the most important thing.
