to uaesales@samba.com along with the front and reverse copy of your valid Emirates ID Card and Passport
copy with visa page. This form also needs to be submitted in original, along with the Samba Application form

• I authorize Samba to enquire and review my credit history held with UAE authorities including the Credit Bureau and Central Bank.
• I authorize Samba to verify all the information provided by me, to conduct reference checks against screening lists and appropriate databases
to assess my credit worthiness.
• I acknowledge that the application reference number assigned by Samba will be a non-transactional account and the account will not be
opened until I have met the designated Samba representative and provided the required documents as per Samba’s Terms and Conditions.
• I acknowledge that any telephone calls between Samba and myself may be recorded and can be used as evidence in any proceedings.
• I acknowledge that the approval provided based on the information in this Consent Form is not final and that Samba, at its sole discretion, can
accept or reject my application for the facility, without assigning any reason whatsoever.
