ADDENDUM TO THE LEASE AGREEMENT

Know All Men By These Presents:

This Addendum to the Lease Agreement is made and entered into on


______________________ at Davao City, by and between:

______________, Filipino, of legal age, and a


resident of Davao City, hereinafter, the
“___________”

_________________________Inc., a corporation
duly organized and existing by virtue of the laws of
the Republic of the Philippines with office address at
_________________, Davao City, herein represented
in this act by its General Manager,
________________, hereinafter, the “Company”

WITNESSETH THAT:

WHEREAS, the parties executed a Lease Agreement on


__________________ (recorded by Notary Public _______________________)
hereinafter referred to as “Lease Agreement.”

WHEREAS, the parties agree to amend the Lease Agreement;

NOW THEREFORE, for and in consideration of the foregoing premises,


the parties have agreed to the following provisions:

1. The parties hereby agree to amend the following provisions to the


Agreement:

“ 3. The weekly rental fee payable by the Entrepreneur to the


Company is $ .05 per box.

2. Any addendum, amendment, or modification to the terms and


conditions herein or to the Lease Agreement shall be in
writing.

3. This Addendum to the Lease Agreement shall take effect upon the
signing hereof.

IN WITNESS WHEREOF, the parties hereto have caused this Agreement


to be executed by them and/or their duly authorized representatives this on the
date and at the place above written.
