1. That I am the biological father of MICHAEL HUI MANONGSONG
who was born on October 12, 1995 in Bindoy (payabon), Negros
Oriental;
2. That his biological mother is MAILEEN ANG HUI;
3. That from the time of conception up to the time of birth of my said
son MICHAEL, her mother MAILEEN and I were not yet married;
4. That I hereby acknowledge that I am the biological father of
MICHAEL HUI MANONGSONG who was born on October 12, 1995
in Bindoy (payabon), Negros Oriental;
5. That as the biological father, I hereby give my consent to my said
son MICHAEL to use my surname MANONGSONG to
appear/enter/record in his birth certificate;
6. That my son shall be known as MICHAEL HUI
MANONGSONG;
