AFFIDAVIT OF WAIVER

I, Candelaria R. Falguera, of legal age, widow, Filipino, with postal address at Blk. 17,
Lot 3 Molave Village Kalipayan St.,Sagkahan Tacloban City,after having been duly sworn to in
accordance with law, hereby depose and say:

That I am included in the NHA qualified applicant /awardee under the National Housing
Authority (NHA) Housing Program.

That I am the owner of right over a certain residential lot situated at Block 17, Lot 3 of
Molave Village Kalipayan St.,Sagkahan Tacloban City;

That considering the fact that I already sell the said lot and no longer interested, I desire
to transfer the same to other person;

That such as, I am waiving all my rights, interest and participation over the same in favor
of Luthy Gay Villamor, likewise of legal age, single, and with residence and postal address at
Brgy. 62 – A, Sagkahan Tacloban City.

That the said Luthy Gay Villamor is willing to assume all obligations with the National
Housing Authority in connection with the above-mentioned lot.

NOW THEREFORE, being a NHA Housing Project recipient-awardee as stated above, I


do hereby waive whatever rights I have over the above mentioned real estate property.

In WITNESS WHEREOF, I have here unto set my hand, this 27th day of May, 2019 at
Tacloban City , Philippines.
