AFFIDAVIT OF UNDERTAKING
I, PATROLMAN ARJAY COMEDIDO DUHAYLO, Filipino of legal age,
and a resident of Pres. Roxas, North Cotabato, after having been duly sworn to in accordance with law, hereby depose and state: 1. In my Certificate of Live Birth issued by the Philippine Statistics Authority (PSA), the date of marriage of my parents indicated is September 15, 1975 (See Annex “A” attached herein). 2. However, in the Certificate of Marriage of my parents also issued by the PSA, the date of their marriage indicated is February 11, 2001 (See Annex “B” attached herein). 3. That the true date of their marriage is September 15, 1975, as indicated in my Certificate if Live Birth. 4. That I hereby undertake to cause the correction of the said error in their Certificate of Marriage by filing the appropriate action before the government agencies or judicial bodies having authority to cause the said change. 4. I am executing this affidavit to attest the truth of the foregoing facts,, to update my legal beneficiary, and for such other purposes it may serve.
