AFFIDAVIT OF NON-ACCEPTANCE

I, MICHAEL U. GARCIA, of legal age, married, Filipino, and a


resident of Zone 9, Poblacion, Laguindingan, Misamis Oriental, after
having been sworn to in accordance with law, do hereby depose and
state, to wit:

1. That I am already a retiree of Philippine National Police having


retired effective August 31, 2020;

2. That I am a bona fide member of the Armed Forces and Police


Mutual Benefit Association, Inc. (AFPMBAI)

3. That as a result of membership of the above-mentioned


association, I was issued an Insurance Policy in which I have
not received personally. Thus, when required by the authority to
produce the said policy, I cannot provide one.

4. That I hereby attest that I have not actually received or accepted


the insurance policy issued by the AFPBAI.

5. That I executed this affidavit to attest the veracity of the


foregoing statements and for whatever legal purpose that this
may serve,

IN WITNESS WHEREOF, I have hereunto set my hand this 21st


day of NOVEMBER, 2020 Cagayan de Oro City, Philippines.
