Dear Dr. Zulueta,

We, Mr. Patrick Jayson A. Lawague and Ms. Rhea Liza O. Divinagracia,
applicants for marriage license, hereby authorized Ms. Emmalyn R. Lubay to
act as our representative to pick up our marriage license document that was
supposed to be picked up last August 10, 2020. We were not able to get our
marriage license document on the said date for the reason that Mr. Patrick
Jayson A. Lawague had to go to Monkayo since his brother died the following
week.
