AFFIDAVIT

I, REYNALDO R. RABARA, legal of age, Filipino and a resident of Sual, Pangasinan,


under oath hereby, depose and say that:

1. I am an Administrative Aide I of LGU- Sual, Pangasinan;

2. I am now processing my Terminal leave;

3. I am voluntarily authorizing the Municipality of Sual to deduct whatever financial


obligation that I have from my retirement benefits;

4. I have no pending criminal investigations or prosecution against me under RA No.


3019;

5. I executed this Affidavit to attest to the truth of all the foregoing statements and for
any legal intent it may best serve.
