EXECUTIVE SUMMARY
AnyPlace MD believes our team will provide a turnkey management solution for three of the City of
Austin’s (COA) COVID-Testing sites. The APMD team has more than 100 years of combined project
management, contracting and staffing experience. APMD is the force-multiplier that the COA needs
to get “America Back to Work” and monitor public health. Fully deployed, our experienced team can
manage three COA COVID-19 drive-thru sites with an estimated throughput of 600-850 tests per day
and provide ad hoc services to nursing facilities and community events as needed.

The Challenge: The 2020 novel coronavirus (now called SARS-CoV-2, causing the disease COVID-19)
epidemic has expanded from Wuhan and throughout China and has been exported to a growing number
of countries around the world. Early efforts have focused on describing the clinical course, counting
severe cases, and treating the sick. In less than a month, the global number of confirmed COVID-19
cases more than doubled from 4.8 million cases on May 18th to more than 8.3 million on June 18th, with
over 2 million cases and 116,862 deaths in the U.S. and is expected to double every five days.

In April, Governor Abbot announced a goal of


30,000 tests per day in Texas. Between May
1st and June 17th, the state has averaged
21,000 tests per day. According to a study
from John Hopkins University, there are
currently 3,560 new positive COVID cases a
day in Texas. One Texan per 1,000 is being
tested and of those who are tested 9.1% are
positive.

Protecting public health is central during the


COVID-19 pandemic and this begins with
early public testing. Quick identification of
cases and treatment for those people and
immediate isolation helps prevent the spread
of the virus. Early testing also identifies
anyone who came into contact with infected
people so they, too, can be quickly treated
and isolated.

Early testing is crucial to treating, isolating, or


hospitalizing people who are infected with the virus. Testing is also important to public health in the
effort to mitigate the spread in communities, aiding investigators, and characterizing the prevalence,
spread and contagiousness of the disease.

APMD BACKGROUND
In 2007, Shane Stevens, founder and CEO of Dental Health Management Solutions (DHMS) founded
APMD with the sole purpose of increasing access to healthcare with the belief that empowering patients
via immediate on-demand telehealth medical services is the most efficient and effective way to improve
patient outcomes and expand access to care while reducing overall health care costs.

In 2019, APMD partnered with MDBox to implement telehealth services for our client’s employees and
customers. Our team operates twenty-four hours a day, seven days a week, three hundred sixty-five
days a year with both immediate on-demand providers and “backup redundancy” overflow medical and
behavioral health providers during high volumes. Our current provider network in Texas consists of over

under the RHRP II contract. In 2019, their logistics team staffed and managed 58 events over 20 states
to serve more than 8,000 servicemembers in one weekend. DHMS has been performing on this contract
for over 10 years.

APMD provides project management, training, quality control, and occupational safety plans for its
employees and ensures all federal regulations are met, which has helped reduce common work-related
issues.

Our network of on-the-ground clinicians is on-call twenty-four hours-a-day, seven days-a-week to


respond to our clients’ needs across the globe. Our team has staffed 8,300 positions and has completed
more than 236,429 medical and dental screenings and treatments for service men and women in the
United States (including Hawaii and Alaska).

MDBox’s telemedicine platform was developed in 2016 to provide cost-effective, quality symptom-
specific, consumer-driven healthcare and wellness to everyone. The MDBox platform consists of a
patient portal, or app, that allows patients to choose their visit type based on their chief complaint,
answer questions about their symptoms, and then have a live video visit with a licensed provider. The
provider conducts the visit and charts the information on the propriety purpose-built EHR (electronic
health record) on the provider portal of the MDBox platform. In addition to improving the telemedicine
experience, Reliant saw the opportunity to enhance telemedicine to provide a higher level of care. The

COVID-19 TESTING EVENTS
Drive-Thru Clinics: Our logistics team has 100+ years of experience performing large scale medical and
dental events for our nation’s armed services. We have tapped into this expertise to execute our drive-
thru event model that encourages social distancing that is consistent with CDC and NIH guidelines.
APMD is currently performing both drive-thru and walk-thru events around the U.S. Our team is more
than capable of performing multiple, simultaneous 600 patient per day COVID-19 screening events with
both PCR and serology tests. All we need is a large drivable space and a wi-fi signal.

We have evaluated the current COA model and are confident we can produce the same great results
with our dedicated team.

FACILITY-BASED TESTING

Nursing Home Testing


Less than 1% of Americans live in nursing homes, yet these facilities account for between 15% and 25%
of Covid-19 cases and half of all deaths from the disease in some states; in Minnesota, nursing home
residents and workers account for 81% of all Covid-19 deaths.

If we hope to contain Covid-19, until we have better treatments or a vaccine, we need a concerted
national testing effort focused on nursing homes. From a public health perspective, they are not only
areas of focused harm but are also wells of infection that may advance the pandemic unless we take
drastic steps.

Nursing homes offer a potential opportunity to
contain the virus in a targeted way that benefits
those who live in them, those who work in them,
and everyone else. If we focus our efforts on testing
all nursing home residents and workers, we may be
able to drive down transmission rates in ways that
have eluded us so far.

The APMD team has worked with nursing homes to


provide various medical and dental services for more than 15 years. We understand the importance of
testing both nursing home residents and healthcare personnel. Our veteran team stays up to date with
CDC Testing Guidelines for Nursing Homes and has providers at the ready to conduct testing within 72
hours of notice. APMD strives to be the premier provider of COVID-19 testing for residents of our
community nursing homes by providing measured, quality, and dignified care.

Austin Prison Testing


The COVID-19 pandemic has proven to be deadly,
rapidly developing, and resource depleting for all sectors
of the society. Within this space, prisoners and
correctional staff share an environment known to
amplify, accelerate, and act as a reservoir for outbreaks
of respiratory diseases. Correctional administrators have
extraordinary power over an institution's disease
response, and guidance and collaboration from the
wider health system will be essential.

More than 500,000 correctional officers and correctional medical staff work in this environment. They
are essential personnel during the COVID-19 response and their professions bring them into direct
contact with a high-risk population daily. They share all the risks of the physical environment as listed
above but are additionally exposed through uncontrolled physical contact as the correctional officers
move prisoners or engage in altercations and as medical staff perform physical examinations and
medical procedures. Prisoners may be unable or unwilling to maintain personal hygiene and may
intentionally expose staff to body fluids in an attempt to transmit diseases. Consistent application of
specific preparation, prevention, and management measures can help reduce the risk of transmission
and severe disease from COVID-19.

The APMD team of testing providers are familiar with the Interim Guidance on Management of
Coronavirus Disease 2019 (COVID-19) in Correctional and Detention Facilities issued by the CDC. Like the
nursing home model, we have provider teams at the ready to conduct testing within 72 hours of notice.

COMMUNITY-BASED EVENTS:
History shows that severe illness and death rates tend to
be higher for racial and ethnic minority populations during
public health emergencies than for other populations.
Addressing the needs of these populations in emergencies
includes improving day-to-day life and harnessing the
strengths of these groups. Shared faith, family, and cultural
institutions are common sources of social support. These
institutions can empower and encourage individuals and
communities to take action to prevent the spread of COVID-19, care for those who become sick, and
help community members cope with stress.

APMD is honored to serve our community, neighbors, and friends. Some do not have the means to
receive much-needed testing to fight this pandemic. We are here to help. In June, APMD partnered with
the Financial Literacy Coalition of Central Texas (FLCCT), the U.S Hispanic Contractor’s Association
(USHCA), and the Austin Latino Coalition (ALC) to host a free COVID-19 PCR and antibody testing site at
the Sacred Heart Catholic Church on Reicher Drive. Two hundred and fifty-three frontline construction
workers in Austin were screened.

AMPD has the experience and understands the nuances (scheduling,


language, technologies) of providing COVID-19 testing services at
community events. Our veteran team is at the ready to conduct
testing within one week's notice.

EVENT STAFFING & TRAINING:
Every one of our event staff members (traffic control, lab runners, clinicians, managers, etc.) are
required to complete training and maintain adherence to proper protocols in the use of PPE following
CDC and OSHA guidelines and will be cleared and trained under 29 CFR 1910.1030 for Bloodborne
Pathogen Exposure standard training. These professionals are qualified, licensed, certified, credentialed,
and/or registered in compliance with applicable state and federal requirements through our
credentialing staff and proprietary software. Once fully trained, their duties are governed by a detailed
job description and our Quality Assurance program. At a minimum we would like our leadership staff to
shadow the COA leadership for one days and the COA staff to monitor our event for three days.

AVAILABLE ANTIBODY TESTING:


Serology COVID-19 Tests: In response to the
Coronavirus epidemic, Healgen Scientific has developed a
COVID-19 specific immunoassay. The COVID-19 IgG/IgM
(Whole Blood/Serum/Plasma) Rapid Test Device utilizes
lateral flow technology that is used for the qualitative,
differential detection of both anti-SARS-CoV-2 IgM and IgG
antibodies. In general, antibodies can be detected 1-3 weeks after infection. This test is intended to
screen patients for COVID-19. Combining RNA and Antibody tests can significantly raise the sensitivity
