Dear Valued Client,
Sagicor Life Inc. has taken another step towards providing you with a superior service experience through
our online claims settlement facility. This simple-to-use facility allows you to use your CariCARE card at any
of the service providers listed below to transact business with immediate claims settlement. You can also
view the list of Providers on our website: https://my.sagicor.com/groupweb/
Here are the simple steps involved when you visit any of these vendors to carry out your transaction:
✓ You will enter a provider’s office and recognize signage indicating online claims settlement is
available at the provider’s office as shown in Illustration1.
✓ Submit picture ID together with your CariCARE card, which already contains your name and plan
information as shown in Illustration 2. Each person covered under the medical plan will have to
provide their own card including dependents in order to access the facility.
✓ The provider will swipe the card, input the services rendered and submit the claim.
✓ The computer system will display to the provider, the total expense, less the deductible (where
applicable), less the payment by Sagicor and the client’s portion of the expense.
✓ You will pay your portion only.
✓ Two copies of the transaction will then be generated: a provider copy which the client must sign and
a customer copy for your record of the submitted claim.
✓ The provider will subsequently receive payment directly from Sagicor.
✓ In the unlikely event of a transactional issue, you may be asked to pay for the service in the
traditional manner.

PLEASE NOTE:
There is NO use of the On-line claims system from 8pm nightly.
If you have a C.O.B. (coordination of benefits) arrangement available to you under another health plan, you
should NOT use this facility. Please pay for the service in the traditional manner and submit the claim to the
primary insurer. (First payer)
It is very important to ensure that you know the benefits of your health plan, once the Provider submits the claim
you would NOT be able to change your mind or have them cancel the claim as they are unable to do so.
If an employee does not adhere to the above or is found using the CariCare card irresponsibly, that employee
would be immediately terminated from the Medical plan.
If a provider does not adhere to the above or is found using the CariCare card system irresponsibly, that provider
would be immediately terminated from the system.

Card Tips for the Insured
• To access the online system you MUST be in possession of your CariCARE card.
• Photo ID MUST be presented to the service provider when using your CariCARE card.
• Medical benefits MUST only be accessed by the cardholder. Dependents will also have a
card in their name.
• A parent or guardian MUST sign the Explanation of Benefits (EOB) on behalf of dependent
children.
• Ensure that you receive an EOB from the service provider upon use of the card, this EOB
must list the services rendered on the day and the actual fee for these services.
• Upon receipt of the EOB, review carefully before signing.
• Sagicor will mail you a copy of the EOB for your records.
• Upon receipt of the EOB from Sagicor, review carefully and notify us of any discrepancies.
• Replacement cards can be obtained for a fee of $25.00.
• Please note the card is not available for swipe when travelling. The swipe facility is only
available in the country in which it was issued.
SECURITY
• Your medical card features a magnetic strip at the back. Please keep safe from magnets
and cell phones as they may adversely affect the functionality of your card.
• Keep your card in a safe place at all times.
• Do not allow anyone to make a copy of your medical card.
• Do not leave your medical card at any provider office.
• Never allow a friend or family member to use your card.
• Should you lose or have your medical card stolen, contact Sagicor immediately.
