1. I am applying for closure of my business under Tax Identification Number
_____________;
2. One of the requirements for the said closure of business are: (a) List of ending of inventory of goods, supplies, including capital good; (b) Inventory of unused sales invoices/official receipts (SI/OR); (c) Unused sales invoices/official receipts and all other unutilized accounting forms;
3. All requirements mentioned above can no longer be found in my office because the business was not operating since 1998;
4. Diligent efforts have been exerted to locate the said documents but the same are already deemed lost;
5. This Affidavit is executed to attest to the truth of the foregoing;
