JUDICIAL AFFIDAVIT OF DR. RONNA RICA CO
I, DR. RONNA RICA CO, Filipino, of legal age, married, and a resident of Brgy.
Canan, Paniqui, Tarlac; after having been sworn in accordance with law depose and state that:
PRELIMINARY STATEMENT
1. That Atty. Jonathan G. Pacifico, a resident of 01 Hilario St., Brgy. Ligtasan, Tarlac Coty
2300 Tarlac, is the counsel who conducted and supervised my examination as a witness
at his law office in 01 Hilario St., Brgy. Ligtasan, Tarlac Coty 2300 Tarlac;
2. That I am answering the questions asked of me based on my personal knowledge, fully
conscious that I am doing so under oath, and that I may face criminal liability for false
testimony or perjury;
OFFER OF TESTIMONY
The testimony of witness DR. RONNA RICA CO is being offered to prove the
allegations contained in the Complaint of Sunshine Melegrito.
PRELIMINARY QUESTIONS
The following are the Questions propounded by Atty. Jonathan G. Pacifico and my
answers in English language:
Question No. 1 : Do you swear to tell the truth and nothing but the truth?
Answer : Yes.
Question No. 2 : Please state your name and your current employment?
Answer : I am Doctor Ronna Rica L. Co, of legal age, married, and a
resident of Paniqui, Tarlac. I am an Obstetrician-
gynecologist and currently working in the Tarlac Provincial
Hospital in Tarlac City, Tarlac.
Question No. 3 : What is your educational background?
Answer : I finished Doctor of Medicine at University of the
Philippines.
Question No. 4 : When did you take and pass physician licensure exam?
Answer : In 1996.
Question No. 5 : Did you pursue any further studies after medical school?
Answer : Yes, I pursued a Residency in Obstetrics and Gynecology
at University of the Philippines – Philippine General
Hospital (UP-PGH) and passed the diplomate exam.
Question No. 6 : So you are a Obstetrics and Gynecology?
Answer : Yes.
Question No. 7 : Where are you practicing your profession?
Answer : In the Tarlac Provincial Hospital at San Vicente, Tarlac
City, 2300 Tarlac.
Question No. 8 : How long have you been practicing your profession as a
Gynecologist?
Answer : I have been practicing my profession for almost 20 years.
Question No. 9 : Is this your first time to testify as an expert witness?
Answer : No, this is not my first time.
Question No. 10 : So with your 20 years of experience in the medical
profession, you can say with absolute sureness that you are
indeed an expert witness and is very much qualified to
testify as to the condition of Sunshine Melegrito at the time
of examination, and the operations that you performed?
Answer : Yes.
Question No. 11 : Have you examined rape victims or persons alleging to be
rape victims before?
Answer : Yes.
Question No. 12 : Have you ever examined a woman by the name of Sunshine
Melegrito?
Answer : Yes. I have examined Sunshine Dela Cruz on February 10,
2020.
Question No. 13 : Could you describe her physical built?
Answer : She was of small built, somewhat 5 feet in height, and of
fair complexion.
Question No. 14 : What were your findings?
Answer : My findings are as follows:
- Deep lacerations at 3 and 12 o’clock positions due to
insertion of a bottle like item.
- Soft tissue swelling 2cm x 2 cm, left thigh, proximal 3rd,
medial aspect.
- Hematoma, left breast, upper inner quadrant.
Question No. 15 : Did you prepare a medico-legal report showing such
findings?
Answer : Yes.
Question No. 16 : I have here a document entitled Medico-Legal Certificate
Report dated February 14, 2020, is this familiar?
Answer : Yes.
Question No. 17 : At the bottom of this document entitled Medical
Certificate, there appears to be a name, Dr. Ronna Rica Co
with a signature over it. Is this your name and signature?
Answer : Yes.
