COMES NOW, the undersigned Notary Public for Cebu City with
Notarial Commission No. 055-19 issued on April 11, 2019, unto the
Honourable and Respectable Executive Judge of the Regional Trial
Court in Cebu City respectfully states that:

1) The undersigned Notary Public was maintaining a regular place


of work or business in Unit 104, 2nd Flr., Marijoy Bldg., F. Ramos
St., Cebu City;

2) Currently, the undersigned counsel has ceased to maintain


office in the place aforecited and is now holding his office at:

“WealthBank- Ayala Branch, Taft


Financial Center, Cardinal Rosales Ave.,
Cebu Business Park, Cebu City”;

3) Pursuant to Section 1, Rule X of the 2004 Rules of Notarial


Practice the undersigned Notary Public hereby submits this
signed and dated notice for the confirmation of the Honourable
and Respectable Executive Judge so that the undersigned
Notary Public can thereafter notarize documents bearing his
new address.

PRAYER

WHEREFORE, premises considered, it is most respectfully


prayed of before the Honourable and Respectable Executive
Judge to confirm herein undersigned Notary Public changed of
regular place of work or business so that he can now notarize
documents bearing his new address.
Other reliefs, just and equitable, under the premises are
likewise prayed for.
