AFFIDAVIT OF NON-FILING OF INCOME TAX RETURN

I, Suzette B. Cabiles, of legal age, Filipino citizen, and residents of


#262 V Dakila Street, Barangay Batasan Hills, Quezon City 1126, after
having been duly sworn to in accordance with law, hereby depose and
state:

1. That I am the mother of Kate Ricxette M. Cabiles;

2. That Kate Ricxette M. Cabiles is currently enrolled as a Grade


7 student at New Era University Integrated School High School
located at No. 9, Central Avenue, Quezon City;

3. Kate Ricxette M. Cabiles is applying in a Scholarship Program


under the FAPE-PEAC-DepEd Education Service Contracting
(ESC) Program;.

4. That one of the requirements is a proof of the Bureau of Internal


Revenue (BIR) validated Income Tax Return (ITR) of
parents/guardian;

5. That I did not file my ITR at the BIR because my income does
not exceed Php 30,000.00, my job being a volunteer worker,
respectively;

6. That I am executing this affidavit to attest to the truth of all the


above facts and statements for all legal intents and purposes it
may serve.
