KNOW ALL MEN BY THESE PRESENTS:

This contract is made and entered between:

________________________________., duly incorporated under the laws of


the Republic of the Philippines, with principal office at, represented by
hereinafter, referred to as the “First Party”

-and-

________________________________, of legal age, Filipino and with


residence address at ____________________, hereinafter, referred to as the
“Second Party”

-WITNESSETH-

1. That the First Party is in need of the services of the Second Party as;

2. That the Second Party has signified his intention, to which the First
Party has accepted, to provide the services needed by the latter;

3. That the Second Party hereby possesses the education, experience


and skills required to perform the job as described herein;

4. That in view hereof; the Second Party is hereby contracted as a

5. In consideration of _______________________ to be paid every first day


of the succeeding month.

6. That as _______________________, the Second Party is expected to


perform the following responsibilities & functions:

7. That the Second Party shall perform work at a time and schedule to
be agreed upon by both parties;
8. That it is understood that this contract does not create an
employer-employee relationship between the First Party and the
Second Party;

9. That the First Party reserves the right to unilaterally terminate this
contract anytime during the contracted period by giving advance
written notice for at least thirty (30) days to the Second Party,
should it find the services of the Second Party unsatisfactory or for
want of capacity, and his services is no longer needed, or for any
breach of terms and conditions of this contract or due to
insufficiency of funds. Termination of this contract as herein
provided shall be final and binding upon the Second Party.

IN WITNESS WHEREOF, the parties have hereunto set their hands this
