Dear Sir;

Good Day!

I, ________________________________________the owner/duly authorized representative of the


land and building both located at ________________________________________________.

Presently, I have entered into a Contract of Lease with Globe Telecom Inc., over the said property, but
the latter requires the submission of a tax declaration as part of documentations needed for the
finalization of the Lease Agreement.

I understand that, as the owner, it is my duty to submit the complete requirements for the issuance of
the said declaration however I am still securing the other documents as a requirement for its issuance.

Further, the uneventful condition of my tax declaration halts the processing and issuance of the building
permit of Globe for the installation, construction and eventual operation and maintenance of Globe Cell
site.

In this regard, I would like to ask from your good office to let this letter be an equivalent to the tax
declaration in order for the Office of the Building Official to process the building permit of Globe. I
promise to submit the complete requirements within ______weeks/months from the execution of this
document.

I am hoping for your consideration and approval on this matter. Thank you.
