AFFIDAVIT OF DISCREPANCY

I, Glennbert Tumali Ferrer, Filipino, of legal age, single, with postal address at Block
6, Lot 29, Legian 2B, Carsadang Bago I, Imus Cavite after having been sworn to in accordance
with law, hereby depose and state:

1. That in my Transcript of Records and Diploma, my middle name was erroneously written as
"(Tumale)" instead of my true and correct middle name of "(Tumali)"

2. That my true middle name is "(Tumali)"; as appearing in my Birth Certificate (PSA), a copy of
which are hereto attached as Annex “A”;

3. That I am executing this Affidavit of Discrepancy to attest to the truth of the foregoing facts and
to explain the discrepancy in my Transcript of Records and Diploma, as regards my true middle
name.

5. That I assumed the name Glennbert Tumali Ferrer, when I signed official documents.

7. That I am executing this Affidavit to clear any discrepancy which may arise from official
documents bearing either name “(Glennbert “Tumale” Ferrer)" or “(Glennbert “Tumali
Ferrer)”.
