Petitioner, LUEL C. MENDEZ, respectfully alleges:
1.
That he is a citizen of the Republic of the Philippines;
2.
That he is over 21 years, having been born on December 27,
1963;
3.
That he is a resident of Block 15, Lot 34, Villa Cacacho Subd.,
Mankilam, Tagum City, Davao del Norte, Philippines;
4.
That he maintains a regular place of work or business at Door 5
& 6, 2nd Floor, SKL Building, Osmea Street, Tagum City, Davao
del Norte, with telephone number (084) 216-1540;
5.
That he is a member of Philippine Bar, having passed the Bar
Examination given in 2002 and admitted to the practice on
March 20, 2003, with Roll No. 47793, Page No. 60, Book No. XX,
attached hereto as Annex A is his Certificate of Membership of
the Philippine Bar; his IBP No. 1039970 and PTR No. 1843541,
was
issued
on
October
26,
2016
and
January
4,
2016
respectively;

That
he
posses
disqualifications for

all
the
qualifications
and
none
of
the
the office of Notary Public;
That he has never been convicted of any crime involving moral
turpitude;

That attached hereto as Annex B, are the three (3) specimens
of his official signature and true impression of his dry seal made
pursuant to law;
9.
That a Certificate of Good Moral Character was jointly issued by
Atty. Gerald B. Racho, Chapter President and Atty. Guineverre Q.
Etulle-Tenaja, Chapter Treasurer of the Integrated Bar of the
Philippines (IBP), Province of Davao del Norte, attached hereto as
Annex C and Certification duly issued by the Supreme Court
that he has no pending administrative case, attached hereto as
Annex D;
10.
That three (3) passport size pictures are also attached as
Annexes E, E-1 and E-2, taken five (5) days before filing of
this petition;
11.
That Official Receipts were issued in favor of petitioner by the
Office of the Clerk of Court, as proof of payment for the filing of
this petition and attached as Annexes F, F-1, F-2 and F-3
hereof.
