STATEMENT OF ALLEGATION UNDER REMOVAL FROM


SERVICE (SPECIAL POWERS) ORDINANCE 2000
You Mr. Muhammad Akbar S/o Sultan Muhammad Security Guard

Shalamar Complex Lahore, are charged with the allegations and other relevant
circumstances.
2.

As Security Guard in Shalamar Complex Lahore, you have


committed the following acts of omission and commissions, amounting to
misconduct: a.

Security Inspector Shalamar Complex reported that you


remained absent from duty w.e.f 14.07.2007 to 02.09.2007 without
any approval / sanction of leave.

b.

You submitted medical certificate which was forwarded to


Medical Superintendent WAPDA Hospital Lahore for verification
and counter signed M.S WAPDA Hospital vide his letter No. HOSP
/ 30593 / MISC / 11077 dated 08.09.2007 and reported that your
medical certificate is not justified to be verified, hence the period be
dealt with administratively.

c.

You remained absent from duty w.e.f 13.09.2007 to 18.09.2007


and 21.09.2007 to 31.10.2007 without approval / sanction of leave.

d.

Your service record reveals that you are ill-disciplined,


inefficient, irresponsible and habitual absentee and miserably failed
to perform your legitimate duties, rather creating administrative
problems in the office as will as for the staff also.

Thus you are found guilty of Misconduct.
