5 Principles of Problem Solving
A problem is an obstacle which makes it difficult to achieve a desired goal, objective or
purpose. It refers to a situation, condition, or issue that is yet unresolved.
There are many approaches to problem solving, depending on the nature of the problem and
the people/environment involved in the problem.
The most common method used in industries to solve the problems related to products is
Root Cause Analysis. This will be discussed in this presentation.
With minor variation there are 2 techniques mainly used in problem solving :

ii) 8 Disciplines(8D) Problem Solving Process or Team problem solving
(The method described here is based on problem in a physical product, but it can be adopted
in other cases on the similar lines)

1. Defining the problem
2. Identify Root Cause
3. Corrective Countermeasure(s)
4. Confirm Countermeasure(s)
5. Feedback / Feedforward
1. Defining the problem
Define the problem objectively by Gathering Data to get as much information as possible
using 5W2H & clarifying the facts about general condition of problem parts/processes (For
details see my previous post)
2. Identify Root Cause
Identify root cause using Brainstorming & Fishbone chart / Cause-&-Effect diagram / Ishikawa
diagram.
Work towards finding root cause by repeating Why?Why?Why? At least 5 times.
Investigate both causes of occurrence(Hard item) & non-detection(soft item).
Summarise root cause.
Clarify why problem was not detected earlier?
3. Corrective Countermeasure(s)
After finding root cause, countermeasure is taken :
1. Temporary till the permanent countermeasure is evolved.
2. Permanent
Countermeasure on hard items should be included even if the problem is attributed to man.
Countermeasure for occurrence & non detection is necessary
Record date of implementation of countermeasure & details
What effects are predicted by Quality inspection data?
Correction- What action were taken on stocks & sold articles?
What should be done if original countermeasure fails?
4. Confirm Countermeasure(s)
After taking countermeasure confirm results of effects at process or market.
Effects to be confirmed by change of Quality data, etc, before & after countermeasure
5. Feedback / Feedforward
Apply Countermeasure to similar problems/similar processes and potential problems of similar
nature.
