PURPOSE
The main purpose of this document Inspection and Test Plan (ITP) is to ensure the quality
control and testing activities are in-placed and performed with quality and safe in a different
activities by the QA/QC inspection personnel of Fatimah Bersatu Sdn Bhd (FBSB) and owners
representative.

2.0

SCOPE
The inspection and test plan covers all inspections, checking and testing activities to be
performed at all activities related to Instrument Installation scope of works such as SCADARTU, Fire & Gas Trip signaling and Process Control System Trip indication status of switchgear and
etcc.
The main scope is to ensure all Instrument installation such as SCADA-RTU, Trip signal of
switchgear, feedback signal to Main PCS, fiber optics are inspected and tested according to
owners company standards and engineering global standard.

WITNESS (W) -

An identified critical step in process whereby an inspection


Activities must be witnessed, inspected, approved and signed by
FBSB inspection team and owners representative as mentioned
in the Inspection Request Form (IRF).
IRF shall be submitted 48 hours before the date of the inspection,
on case-to-case basis and agreed both FBSB and Owners
representative the Inspection activity may proceed with or
without OWNERs representative.

HOLD (H)

The same as Witness (W) except that no further work can


proceed on the affected item, or part of an item until the work
has been inspected and approved through Inspection Request
Form (IRF) that shall be submitted 48 hours before Inspection
to be carried out.

3.10

SPOT WITNESS (S)- Also known as Surveillance, a work in progress is allowed to


continue but where an FBSB inspector and owners
representative may invoke Witness or Hold attribute as
necessary.

3.11

INSPECTION (I)

The process of checking and verifying on physical which occurs


parallel with flow of work and carried out according to Project
Procedures Method of Statement code and standard etc.

3.12

RECORD (RC)

FBSB inspection personnel shall record all necessary


parameters, datas and results which have been inspected or
witnessed into the relevant documents as required and
witnessed by owners representative

3.13

REVIEW (R)

An activity that occurs in parallel with normal flow or work


applied to cross checking and verify previous obtain data
reports certificate test record etc.
