I, Isabel Tee, of legal age, single, and resident of 105 Banal St. 6th Avenue Caloocan City, after being duly sworn according to law, hereby depose and state that:
1. I am the Custodian of DR No. 128 with the amount of P 17, 740.80
and DR. No. 134 also with the amount of P 17, 740.80 issued for Alexis Mktg. Inc., by Finco Sales Corp. on November 29, 2014 and December 2, 2014 respectively at 1534 Benitez St. Malate, Manila; 2. Sometime in December 16, 2014, the above mentioned DRs were lost under the following circumstances: That on or about December 16, 2014 around 1am in the morning, some unidentified men entered Finco Sales Corp. Manila office and stole its safety box which was placed inside said office. Inside the safety box were invoices and counter-receipts of different customers of Finco Sales Corp here in Manila branch for collection. The said incident was reported to the nearest Barangay office for blotter the following day. 3. Despite diligent efforts exerted, the company could no longer locate, find or recover the DR No. 128 and DR No. 134, and to the best of my knowledge, the same is truly lost. 4. The said delivery receipts has not been confiscated by any officer of the law or the issuing office or agency due to the violation of any law, statute, order, rule or regulation. 5. In the event that the delivery receipts should hereafter be found or located, I undertake to forward or surrender the same to Finco Sales Corporation for appropriate disposal. 6. I am executing this affidavit to attest the truth of the foregoing statements and to request the issuance of a new invoice and counter-receipt in lieu of the one which was last. Manila office and stole its safety box which was placed inside said office. Inside the safety box were invoices, counter-receipts and sales invoices of different customers of Fanbo Industrial Co. here in Manila branch for collection.
