Guidelines for the Narrative Report – Advanced Nursing Related Learning Experience-From
theory to Praxis
Course Requirements:

- short bond paper, use Times New Roman, size 12
- margins - 1 ½ inch on the left, 1 inch on all other sides
- pagination should be found on the lower right part
Parts of the report
Part 1. Executive Report
1. 1 ABSTRACT- write a 300 word abstract with keywords.
1.2 The executive summary (ES), often the most influential part of the report, should be
written with the busy transportation administrator in mind. The ES should provide a readable,
accurate, and condensed description of the report, findings and conclusions that evolved from
the project. The ES should contain only information essential to understanding the findings and
how they relate to the solution of problems–It is NOT an abbreviated version of the full report.
An Executive summary summarizes the content of the different sections of a report
which is situated before the main content of a report. The executive summary allows the
reader of a report to scan the most import elements of a full length report. It provides the
main contents of a report in a nutshell and is the first part of a report that a reader will
read. (ONE PAGE)
Part 2. HOSPITAL profile that includes Vision, mission & philosophy; core values, organizational
structure, duties and functions of the administrative officers , chief nurse, asst. chief nurse,
senior staff, staff nurses, etc.
Part 3. Case analysis for diabetes, cardio, and oncology cases. Select 3 cases and decide among
students which case to present for the case colloquium. Note: Variety of cases is suggested that
represents the 3 nursing specialties.
The following are the parts of the case analysis:
1. Introduction:
Answer the following questions:
- Why choose the case in particular?What is the relevance of the case as to
incidence/prevalence?
- Include an overview of the case then discuss comprehensively the advance pathophysiology
of the case.
2. Patient’s Profile
- Includes personal data, chief complaints, assessment upon admission, date of admission,
Nursing History – History of present illness, other data ( lab findings, dx tests, special
procedures, etc) physical assessment during the case analysis utilizing any physical assessment
tool e.g. Gordons’ PERSONS, etc.
3. Nursing Care Plan
- Give only at least 5 priority care plan for the client which was implemented by the student ,
maximum number of care plan -5 nursing problems
- Format is as follows:

4. Drug Study – explain why these drugs were indicated. Discuss nursing responsibilities/drug
alerts, etc.
5. Reflective journal or Journal Log:
The following are the parts of the Journal Log

Reflections/ Insights about the exposurereflective Journal which is based on Jean
Watson’s caring theory) transpersonal caring relationship, caring
moment/occasion, 10 caritas processes, or use any caring theory deemed relevant
as framework of praxis.
4.1 Guide questions: Reflective Journaling:
What did I learn ? How did I feel about it? How will I use the learning?

Submission: One week after the case colloquium.
Case Colloquium is scheduled a week after commencement of practicum exposure.
Schedule of Comprehensive exams (February for those graduating in March; September for those
graduating on October.
