Agency provides rehabilitation, vocational and residential services for children and adults. ELIGIBILITY: Individuals must qualify for waiver services or be assessed for services.

Provides medication, physician visits and follow-up, some physical and occupational therapy, equipment, surgeries and bracing, hearing, speech and language evaluations. Speech and language therapy available to clients not enrolled in clinic. SERVICE AREA: Breckinridge, Grayson, Hardin, LaRue, Marion, Meade, Nelson and Washington counties. The services are not limited only to these areas. There are 13 locations in Kentucky. ELIGIBILITY: Children age birth to 21 years of age. Must live in Kentucky. Must qualify diagnostically. FEES: Sliding fee based on income and family size (special consideration for outstanding medical bills). Accepts insurance and medical assistant card.

The mission of the Center is to help children and adults with disabilities achieve maximum independence. Our programs are designed to meet the ever changing needs of the community. OFFICE HOURS: 8:00 AM -5:00 PM (Monday-Friday) SERVICES: Speech therapy, pediatric occupational and physical therapy, audiological services, hearing aid dispensing, developmental intervention. SERVICE AREA: Jefferson, Hardin, Shelby, Oldham, Spencer, and Nelson counties. ELIGIBILITY: No specific eligibility requirements. FEES: Fees vary for different programs. A sliding scale fee based upon the person's ability to pay is available. Medicaid and third party payors are also accepted.

Services are aimed toward assisting the person in entering into employment. Services include guidance, counseling, evaluation, treatment, training, and job placement. SERVICE AREA: Nelson County ELIGIBILITY: Based on the presence of a disability which is an impediment to employment FEES: No fee for diagnostic, evaluation, counseling or placement. Other services may be based on financial need

SERVICE AREA: Hardin, LaRue, and Meade Counties. ELIGIBILITY: The customer must have a physical or mental impairment. The impairment must result in a substantial impediment to employment. If the above two criteria are met, the customer will be determined eligible unless there is clear and convincing evidence that the customer cannot benefit from or does not require vocational rehabilitation services. An eligibility decision will be made within 60 days. FEES: None
