SUBJECT: Memo re Administrative Hearing


Dear Ms. Del Tomas,
Our records show that you have not reported for work since 5 June 2006 without any prior
notice, permission or explanation on your part. On several occasions, you have been advised
verbally that your performance in the work is not acceptable. However, despite these
warnings, your behavior did not improve.
As a consequence of your unexplained absences and misconduct, you have failed to
satisfactorily discharge your duties, thereby exposing you to the commission of gross and
habitual neglect of duty which as per Article 282 of the Labor Code is a ground for the
termination of your employment
As you very well know, these acts, if not satisfactorily explained, violate company policies
and your duties as Therapist, not to mention that of the Labor Code.
In view thereof, you are directed to appear at an administrative hearing on June 23, 2006,
2:00 P.M., at the Conference Room of Nurture Tropical Spa, Pulong Sagingan, Barangay
Maitim II West, Tagaytay City to explain why no disciplinary action should be taken against
you, including termination. At the administrative hearing, you will be expected to submit
your written explanation for your acts and to explain why no disciplinary action should be
taken against you. Your failure to give your written explanation within the period stated and
to attend the hearing will be construed as a waiver on your part of your right to be heard.
Accordingly, your case will be resolved based on the available records and evidence.
