Gout (Niqras) is a common metabolic disorder which causes inflammatory arthritis generally presenting as monoarthritis. It is a true crystal deposition disease caused by formation of monosodium urate crystals hinge joints and other tissues. Unani classical text provides a comprehensive detail on the concept of Niqras. According to Ibn Sina Niqras is a type of pain which starts sometimes from fingers, toe, and sometimes heel. Owing to dreadful complications of Gout and lack of safe and effective drug for its management, it becomes a thrust area for research, in every field of medical science. Unani medicine has a variety of safe and effective drugs for the management of Niqras. And Unani medicine claims have been successful in treating this disease since a long time without any unwanted effects.

Background: The conventional drugs used for gout are full of adverse effects (hepatotoxicity, nephrotoxicity, haematological toxicity). A typical excruciating type of inflammatory arthritis, Gout, often affects big toe joint and sometimes other joints and viscera as well. In Greco-Arab (Unani) medicine gout is known as Niqris. There is ample literature regarding gouty arthritis available in the treatise of Greco-Arab medicine, an age old existing medicinal system. The aim of this study is to explore the medicinal treasure for gout in Greco-Arab medicine, correlate it with the modern science to amplify the knowledge and widen the field of research avenues. More efficient, cost-effective management of gouty arthritis with least side effects could be achieved with these medicines.

Unani Dieto-Therapy Approach with Special Reference of Lifestyle Modifications to Manage Niqras (Gout): An Old Metabolic DisorderNiqras (gout) is one of the oldest known diseases and the most common form of inflammatory arthritis, resulting from the deposition of monosodium urate crystals in the joints and various other tissues, causing pain and swelling in the affected areas. The main causes of primary gout appear to be dietary and it's thought to occur when the human body makes too much uric acid and doesn't excrete enough of it. Since the immemorial, gout has been associated with overeating, drinking, and wealthy men who overdo it with food and drink. Overeating (ghaleez ghiza e.g. red meat, seafood), obesity, alcohol consumption, excessive coitus especially when stomach is full, indigestion were recognized as contributing factors even in ancient times. Consumption of sugar sweetened soft drinks sweet, and fructose, which had not been previously recognized, was described as a new risk factor for gout. Finally, protective factors such as skim products, high dose vitamin C and caffeinated coffee have also been described. About 70% of uric acid is produced by human body, with the remaining 30% coming from foods and beverages that contain purines. Gout is treated with medications, but a healthier diet can play an important role in treating hyperuricemia various associated diseases. A low-purine diet used to be recommended to manage gout and maintaining a healthy weight and eating a healthier diet are the best ways to treat gout. The goal of this article was to provide diet and lifestyle recommendations for gout and hyperuricemia that were developed by a group of rheumatologists; based on a review of the most recent literature.

Evaluating the Role of Suranjan in Combination with Sharbat-e-Bazoori for the Management of GoutGout is metabolic disease that usually affects middle age and rarely elderly people. Clinically it is observed that this disease occurs due to intake of protein rich and high calorie diet. Pathologically deposition of mono-sodium urate crystals in joint space and other tissues results in the symptoms like pain and inflammation. Methods: in this study, patients of gout are treated with soranjan tablet in combination with sharbat-e-Bazoori for a period of one month who qualified the inclusion criteria. Results: we observed that severity of joint swelling, pain, inflammation, tenderness, stiffness and mobility resolved significantly after the treatment (p-value <0.0001*). All the objective parameters like; Sr. uric acid, ESR and CRP improved significantly after the treatment with a p-value (<0.0001*). Conclusion: owing to the painful and stressful condition of the gout, it has been demonstrated that a combination of two Unani drugs (Habi Suranjan and Sharbat-e-Bazoori) together effectively treats gout and the associated symptoms.

Hyperuricemia is considered to be one of the leading causes of joint stiffness and pain especially small joint most commonly affecting first metatarsophalangeal joint. This is an excess of uric acid in the blood. It is commonly associated with traditional risk factors such as dysglicemia, dyslipidemia, central obesity, gout (Nikras), gouty arthritis, tophi formation, abnormal blood pressure, etc. Concordantly, recent studies have revived the controversy over the role of circulating uric acid, hyperuricemia, and gout (Nikras) as an independent prognostic factor. In this regard, the role of Unani compound medicine including Habb-e-Asgandh, Habb-e-Suranjan, and Sharbat Bazoori Motadil in combination with and without Hijama Bil-Shart (Wet Cupping), evaluated on multiple patients with the possibility of lowering increased serum uric acid level. In this review, we will focus on controlling of hyperuricemia with the use of Unani Medicine and Hijama Bil Shart.
Gout is arthritis caused due to Monosodium urate (MSU) crystals deposition occurring particularly in patients with associated comorbidities limiting the use of conventional therapies. This study was planned to evaluate the therapeutic efficacy of urinile (a herbal drug) for the treatment of gouty arthritis. Allopurinol was used as standard drug (positive control). The study population of 250 volunteers (gouty arthritis patients) were divided into 2 groups as test and control group (n = 125 each). Gouty arthritis patients in test and control group were treated with 300 mg each of urinile and allopurinol, respectively. Clinical symptoms of all the study volunteers were recorded and serum uric acid was determined. Significant (p < 0.05) reduction in serum uric acid level toward normal was found in test group individuals.
