Basic pediatrics approached in the format of questions and answers

Is the field of pediatrics doing all that it can for the health of children? Many think that the field has gone off track and that we could do better. These articles highlight some different ways of judging the successes and failures of pediatrics in the United States today. The choices that we face are stark. To change the system, we would need to recalibrate the balance between utilitarian approaches to resource allocation, such as might be dictated by cost-effectiveness analyses, and deontologic approaches that prioritize unbreakable commitments or promises to individuals. These changes would entail large-scale social engineering projects to reshape our health care system, our educational system, and our public health system. A failure to change, however, might perpetuate a system that is not doing all it can for the health of America’s children and the society they will form.

As healthcare reform initiatives place increased emphasis on primary and preventive care, the healthcare industry is likely to experience increased demand for pediatric services in the coming years. The healthcare marketplace continues to experience dramatic change as the business of healthcare becomes increasingly competitive and providers face decreases in reimbursement and changes in utilization levels. Despite their historically low compensation (as compared to other physician specialties), in recent years, pediatricians have seen increases in compensation levels, including salary increases and other amenities, e.g., sign-on bonuses and resident stipends. 1 With the new payment structures and models of care being implemented under the Patient Protection and Affordable Care Act (ACA), reimbursement rates are also expected to rise, potentially signaling a “new day” for these, as some have described, often undervalued providers.

Although the future of pediatrics is uncertain, the organizations that lead pediatrics, and the professionals who practice within it, have embraced the notion that the pediatric community must anticipate and lead change to ultimately improve the health of children and adolescents. In an attempt to proactively prepare for a variety of conceivable futures, the board of directors of the American Academy of Pediatrics established the Vision of Pediatrics 2020 Task Force in 2008. This group was charged to think broadly about the future of pediatrics, to gather input on key trends that are influencing the future, to create likely scenarios of the future, and to recommend strategies to best prepare pediatric clinicians and pediatric organizations for a range of potential futures. The work of this task force led to the development of 8 “megatrends” that were identified as highly likely to have a profound influence on the future of pediatrics.
