Coronary Artery Fistulas: A Review of the Literature and Presentation of Two Cases of Coronary Fistulas with Drainage into the Left Atrium2007 •
We report 2 cases of infants presenting with a murmur shortly after birth and diagnosed with coronary artery fistulas with drainage into the left atrium. The first infant had a fistulous communication between the left main coronary artery and the left atrial appendage and presented with signs and symptoms of heart failure. The infant was repaired surgically in the first week of life. The second infant was asymptomatic and had a fistulous communication between the right coronary artery and the left atrium. The infant will have the fistula closed in the cardiac catheterization laboratory when the child is older. The literature on coronary artery fistulas is reviewed, and the diagnosis and management of coronary artery fistulas is discussed.

Coronary artery fistula is an exceedingly uncommon congenital disorder characterized by fistulous communication of a coronary artery with asystemic or pulmonary vessel or a heart chamber. Small fistulas are asymptomatic and benign while large fistulas present in a variety of ways like heart failure, arrhythmias or endocarditis. We report here a rare case of eight years old girl who presented to us with history of shortness of breath and palpitation. Her echocardiography revealed a moderately large right coronary artery fistula that was opening into right ventricle. Angiography later confirmed its presence. We surgically closed it with a pericardial patch on cardiopulmonary bypass. The girl showed uneventful recovery and her postoperative echocardiography revealed no residual fistula. She was discharged on 5th postoperative day and her follow up was also unremarkable.

Coronary Artery Fistula: Single-Center Experience Spanning 17 Years2007 •
Coronary artery fistula (CAF) is an uncommon form of congenital heart disease. It is often diagnosed incidentally during angiograms. We have reported on clinical characteristics, diagnosis, and management of CAF. Retrospective review of a tertiary referral institution's database identified 30 patients with CAF between 1987 and 2004. Mean follow-up was 31.61 +/- 48.03 months. Mean age was 60 +/- 12.7 years. Most common site of CAF origin was the left anterior descending artery (LAD) (14, 46.7%). The most common site of drainage was the main pulmonary artery (15, 50%). Therapeutic strategies were based on symptoms and shunt size. Conservative management was the option in 17 patients (56.7%) with small shunts and mild or no symptoms. Patients with moderate/severe symptoms and/or large shunts were treated with either percutaneous embolization (6, 20%) or surgical ligation (7, 23.3%). Four patients (13.3%) died during follow-up. No deaths were reported in the embolization group, two patients died of cancer in the conservative management group, and two patients died in the surgical group due to cardiac tamponade and cancer, respectively. Origin of CAF was predominantly from the left system. Clinical presentations were variable depending on type, size of fistula, and the presence of other cardiac conditions. Management of CAF is still controversial and treatment of adult asymptomatic patients with nonsignificant shunting is still a matter of debate. Newer imaging modalities may enhance noninvasive diagnosis. A national registry is necessary for further insights into optimal treatment for large fistulae and the natural history of smaller fistulae.

Убиство четничког војводе Глигора Соколовића 1910. и српска јавностУбиство српског четничког војводе Глигора Соколовића јула 1910. гоине изазвало је бурну реакцију српске јавности. У многим градовима Србије одржавани су парастоси и митинзи против убица. На њима су за убиство оптуживани Бугати и младотурски режим у Османском царству. Парастоси и пригодне свечаности које су у част убијеног војводе приређивал српска патриотска друштва и организације претворили су се у масовне манифестације које су позивале српско друштво да се припрема за ослобођење Старе Србије. Српска дневна штампа врло је помно пратила истрагу убиства, митинге и манифестације, што је утицало на став српског друштва према младотурском режиму у Османском царству, према потреби поновног отпочињања српске четничке акције и односа Србије према Бугарској и Турској. Рад је написан углавном на основну дневне српске штампе, објављених извора српског и бугарског порекла и необјављене српске грађе.
