Vaccination can mitigate these risks but only if safe and effective in MS patients, including those taking disease-modifying drugs. Methods: A modified Delphi consensus process (October 2017–June 2018) was used to develop clinically relevant recommendations for making decisions about vaccinations in patients with MS. A series of statements and recommendations regarding the efficacy, safety and timing of vaccine administration in patients with MS were generated in April 2018 by a panel of experts based on a review of the published literature performed in October 2017. Results: Recommendations include the need for an ‘infectious diseases card’ of each patient’s infectious and immunisation history at diagnosis in order to exclude and eventually treat latent infections. We suggest the implementation of the locally recommended vaccinations, if possible at MS diagnosis, otherwise with vaccination timing ...
We investigated the potential anti-HIV-1 activity of the candidate microbicide 5-hydroxytyrosol (... more We investigated the potential anti-HIV-1 activity of the candidate microbicide 5-hydroxytyrosol (5-HT) both in primary human cervical tissue explants (CTE), established from tissues of women undergoing histerectomy, and in endometrium-associated leukocytes (EAL). CTE were exposed to either the laboratory-adapted HIV-1BaL or to primary viral isolates in the presence or absence of 5-HT or 3TC/lamivudine as control and were then monitored for 12 days in terms of HIV-1 p24 Gag antigen production in culture supernatants. HIV-1BaL replication was also evaluated in EAL by reverse transcriptase (RT) activity. The highest nontoxic concentrations of 5-HT (200 and 100 μM for CTE and EAL, respectively) exerted a significant inhibitory effect on virus replication in both primary cell systems. 5-HT did not cause significant alterations of the activation profile of CD4(+) and CD8(+) T cells, in terms of CD4, CCR5, CD25, CD69 and HLA-DR expression, although it decreased the percentage of CD38(+)CD8...
ABSTRACT Human papillomavirus (HPV) is definitely recognized as the necessary cause for the devel... more ABSTRACT Human papillomavirus (HPV) is definitely recognized as the necessary cause for the development of cancer of the uterine cervix and the detection of HPV-DNA in cervical samples is demonstrated to own a significantly higher sensitivity towards preneoplastic lesions than conventional cytology (Pap test). Screening, management of atypical Pap tests and follow up of treated patients, represent the optimal settings where HPV-DNA testing has been demonstrated of clinical value. Atypical Pap tests account for cases in which the cytological alterations cannot clearly be attributed neither to negative nor to positive cytology; in these cases HPV-DNA testing has been demonstrated to have a sensitivity very close to 100% in identifying patients with an histologically proven intraepithelial preneoplastic lesion of high grade (CIN2-CIN3). Despite this, specificity of HPV-DNA positive testing lacks of significance and the referral rate to second-level colposcopy is too high. Different options have been tested to improve the specificity and the overall performance of HPV-DNA testing in cases of equivocal cervical cytology; the present paper aims to collect and present data from the recent literature, in order to better clarify the present state of the art in this particular aspect of cervical cancer prevention.

The patient's scratching often worsens the situation and makes diagnosis by the clinician difficult. A clinical trial to assess the safety and efficacy of a topical antihistaminic drug (oxatomide) was carried out. The first stage aimed to determine the best formulation and concentration: eleven patients were admitted in the study, conducted openly. Two preparations (cream and gel) and two concentrations (2.5% and 5%) were tested. A second stage was performed to assess the efficacy of oxatomide gel 5% versus placebo: thirty patients entered a double-blind, cross-over, placebo-controlled study. Results of the first stage demonstrated good local tolerability of the medication, good patient acceptance and no side effects. During the second stage better anti-itching action of topical oxatomide than placebo was obtained. Safety and acceptability were confirmed. In general topical oxatomide showed good tolerability and efficacy in women with vulvar itching of various natures.
