Claims (1)
- 【特許請求の範囲】 1.(a)器具本体を患者の近位脛骨に取り付ける搭載手段を有する器具本体と 、 (b)前記搭載手段は、患者の骨髄内管に通すことができ、患者の骨髄内管に関 して外科医が外科的切除の基準とすることを可能にするロッドを備え、 (c)前記器具本体は、患者の近位脛骨の切除中に外科医の切除ブレードを案内 する少なくとも1つの切除ガイドを有し、 (d)器具本体によって支持され、患者の内側方と外側方のいずれかの関節丘面 で切除の深さを設定する手段とからなる脛骨補綴具を収納するために患者の近位 脛骨を切除する脛骨切除ガイド器具。 2.前記器具が1つの切除ガイドを備える請求項1記載の脛骨切除ガイド器具。 3.前記器具が2つの切除ガイドを備える請求項1記載の脛骨切除ガイド器具。 4.前記切除ガイドが複数の切除ガイドスロットからなる上記請求項のいずれか 1つに記載の脛骨切除ガイド器具。 5.前記器具本体がほぼU字形状である上記請求項のいずれか1つに記載の脛骨 切除ガイド器具。 6.前記複数の切除ガイドスロットが前記ロッドを横断する面を形成する請求項 4または5記載の脛骨切除ガイド器具。 7.前記器具本体が前記切除の深さを設定するスタイラス手段 からなる請求項1、2または6記載の脛骨切除ガイド器具。 8.前記スタイラス手段がスタイラスプローブからなる請求項7記載の脛骨切除 ガイド器具。 9.前記器具が一対のスタイラスプローブで提供される請求項8記載のの脛骨切 除ガイド器具。 10.外科医が同様に切除の深さを設定するように、選択されたプローブの位置 を設定する手段をさらに備えてなる請求項8または9記載のの脛骨切除ガイド器 具。 11.前記スタイラスプローブが患者の近位脛骨に対しその長手方向に沿って調 整可能に接続可能である請求項8〜10のいずれか1つに記載の脛骨切除ガイド 器具。 12.前記プローブが切除の深さを選択的に規定するために器具本体に対して調 整可能で、かつ互いに調整可能である請求項8〜11のいずれか1つに記載の脛 骨切除ガイド器具。 13.器具本体に対する選択されたプローブの調整が、選択されたプローブと切 削ガイド面との間隔の調整を含む請求項8〜12のいずれか1つに記載の脛骨切 除ガイド器具。 14.プローブが器具本体に対し独立して調整可能である請求項8〜13のいず れか1つに記載の脛骨切除ガイド器具。 15.前記プローブがほぼ平行である請求項8〜14のいずれか1つに記載の脛 骨切除ガイド器具。 16.前記器具本体が各プローブと切除ガイドとの間隔を示す表示手段をさらに 備えてなる請求項8〜15のいずれか1つに記載の脛骨切除ガイド器具。 17.各スタイラスプローブがその上に歯の付いたラックを有し、器具本体がそ のラックの選択された歯にかみ合う手段を有する請求項8〜16のいずれか1つ に記載の脛骨切除ガイド器具 18.プローブの長手方向に沿う選択された位置で、器具本体にスタイラスプロ ーブを固定する手段をさらに備えてなる請求項8〜17のいずれか1つに記載の 脛骨切除ガイド器具。 19.第2の切除ガイドが使用中に脛骨に埋め込まれる請求項3記載の脛骨切除 ガイド器具。 20.使用中に使用者が第2の切除ガイドを脛骨内に案内することが可能なきぬ た骨部をさらに備えてなる請求項19記載の脛骨切除ガイド器具。 21.第2の切除ガイドが、患者の近位脛骨の多孔質組織に切り込むことが可能 なブレード部材を有する請求項19または20記載の脛骨切除ガイド器具。 22.前記ブレード部材が第1の切除ガイドとで平面を形成する請求項21記載 の脛骨切除ガイド器具。 23.前記ブレード部材が放射状に配置された複数のブレード部材からなる請求 項21または22記載の脛骨切除ガイド器具。 24.前記ブレード部材が前記ロッド軸を横切る平面を占める21〜23のいず れか1つに記載の脛骨切除ガイド器具。 25.前記ブレード部材が遠位切除面と近位切除面を有し、近位切除面が第2の 切除ガイドである請求項21から24までのいずれか1つに記載の脛骨切除ガイ ド器具。 26.(a)患者の脛骨骨髄内管を穴あけし、 (b)その骨髄内管にロッドを配置し、 (c)患者の近位脛骨上のロッドに、近位脛骨の切除中に鋸子を案内する横断切 除ガイド面と切除開始前に近位脛骨を基準にすることのできる調整可能なスタイ ラス部材を備えたスタイラスとを有する請求項7〜18のいずれか1つに記載の 器具を搭載し、 (d)スタイラスで切除の深さを設定し、 (e)近位脛骨面にスタイラスを合わせ、 (f)鋸子を切除ガイド面に沿わせることにより、鋸子で近位脛骨を横切るよう に切除し、 (g)請求項7〜18のいずれか1つに記載の器具とそのスタイラスを取り外し て、 (h)患者の近位脛骨上のロッドに、近位脛骨に縦方向に切り込むカッターを有 する請求項19〜25のいずれか1つに記載の器具を搭載し、 (i)その第2の切除器具を用いて近位脛骨上に第2の切除を行うステップから なる脛骨インプラントを収納するために外科用鋸子と切除器具で患者の近位脛骨 を前処置する方法。 27.ステップ(i)が、ロッドに対してほぼ垂直な切除を行うことからなる請 求項1記載の患者の近位脛骨を前処置する方法。 28.ステップ(i)が、ロッドに対してほぼ垂直なブレードで切除を行うこと からなる請求項1記載の患者の近位脛骨を前 処置する方法。 29.ステップ(i)が、平らで、かつ第2の切除ブロックとロッドとを組み立 てたときにロッドと交差する平面を有するブレードを備えたカッターで切除を行 うことからなる請求項1記載の患者の近位脛骨を前処置する方法。 30.ステップ(i)が、複数の平面ブレードを備えたカッターで切除を行うこ とからなる請求項1記載の患者の近位脛骨を前処置する方法。 31.ステップ(d)が、第1の切除ブロックの切除ガイド面を、スタイラスの 下端よりも下方に所定の距離をおいて位置付けることにより、スタイラスで切除 の深さを設定することからなる請求項1記載の患者の近位脛骨を前処置する方法 。 32.ステップ(c)が、第1のカッティングガイドを支持する梁を有するスタ イラスをロッド上に搭載することからなり、 前記請求項7〜18のいずれか1つに記載の器具が、近位脛骨の切除中に鋸子を 案内する横断切除ガイドスロットを有し、第1の切除ガイドが、梁上で、ロッド からの距離が変化する位置に調整可能である請求項1記載の患者の近位脛骨を前 処置する方法。 33.ステップ(e)とステップ(f)との間に、ロッドにスタイラスを留める ステップをさらに備えてなる請求項1記載の患者の近位脛骨を前処置する方法。 34.前記ステップ(i)が、請求項7〜18のいずれか1つに記載の切除器具 を用いて近位脛骨上に第2の切除を行い、さ らにその第2の切除を行うときに鋸子を第2のカッターに位置させることからな る請求項1記載の患者の近位脛骨を前処置する方法。
