Description
本発明は、患者の体内に移植される人工心臓弁を対象とする。特に、本発明は、互いに組み立てられる複数部品の心臓弁アセンブリと、複数部品の心臓弁アセンブリの部品の組立て及び/又は移植をガイドする及び/又は容易にする装置及び方法と、を対象とする。また、本発明は、ガイド及び/又は他の部品を含む心臓弁アセンブリの部品を組み立てるための装置及び方法も対象とする。

Claims (31)
- 生体の弁輪内に移植可能な心臓弁アセンブリであって、
前記生体の弁輪内に移植可能な環状部材と、前記環状部材から外側に伸びる縫合カフとを含む第1のプロテーゼと、
環状フレームと、少なくとも1つの弁要素とを含む第2の弁プロテーゼと、
前記第1のプロテーゼに取り外し可能に取り付けられ且つそこから上方に伸びる1つ以上のガイドシールドであって、前記1つ以上のガイドシールドは、前記第2のプロテーゼがそこを通って前記第1のプロテーゼに向かって方向付けられる通路を、少なくとも部分的に規定している前記1つ以上のガイドシールドと、を含むことを特徴とする心臓弁アセンブリ。 - 前記第1のプロテーゼが、前記環状部材及び前記縫合カフのいずれか一方から伸びている複数の細長いリーダーを含み、

前記第1のプロテーゼの周囲に部分的に伸びた幅広のベース部と、
狭い上端部と、を含むことを特徴とする請求項12に記載の心臓弁システム。 - 前記ベース部は、1つ以上の縫合糸によって前記環状プロテーゼに取り外し可能に取り付けられており、前記縫合糸は、前記ベース部を前記環状プロテーゼから分離するのを可能にするために切断されることを特徴とする特徴とする請求項13に記載の心臓弁システム。
- 前記ガイドシールドが、前記環状プロテーゼから離れて配置されるように、且つ前記環状プロテーゼに対して方向付け可能で弁プロテーゼを前記環状プロテーゼに向けてガイドするための前記通路を少なくとも部分的に規定するように、前記ガイドシールドは、1つ以上のつなぎ紐によって前記環状プロテーゼにスライド可能に取り付けられていることを特徴とする請求項12に記載の心臓弁システム。
- 近位端を含む細長いシャフトと、
前記生体の弁輪に導入するのに適した寸法にされた遠位端と、
前記環状部材を前記緩和された状態から前記縮められた状態に変形するためのアクチュエータと、
を含むデリバリーツールをさらに含むことを特徴とする請求項12に記載の心臓弁システム。 - 前記緩和された状態が、略円形の形状を含み、
前記縮められた状態が、複数の小葉状の形状を含むことを特徴とする請求項12に記載の心臓弁システム。 - 弁プロテーゼを前記環状プロテーゼに向かってガイドするための、前記環状プロテーゼから伸びたている複数の細長いリーダーをさらに含むことを特徴とする請求項12に記載の心臓弁システム。
- さらに弁プロテーゼを含み、
前記リーダーは、前記弁プロテーゼを前記環状プロテーゼに向かってガイドするために、前記弁プロテーゼを通って受容可能であることを特徴とする請求項18に記載の心臓弁システム。 - 前記弁プロテーゼは、前記リーダーの各々をスライド可能に受容するための複数のレセプタクルを含むことを特徴とする請求項19に記載の心臓弁システム。
- 前記レセプタクルと前記リーダーとが、前記弁プロテーゼを前記環状プロテーゼに固定するための協働するコネクタを含むことを特徴とする請求項20に記載の心臓弁システム。
- 前記ガイドシールドは上端部を含み、
前記上端部は、前記リーダーを前記ガイドシールドに解放可能に保持するための要素を含んで、前記ガイドシールドによって少なくとも部分的に規定された前記通路から前記リーダーを離れた状態に維持することを特徴とする請求項18に記載の心臓弁システム。 - 前記要素は、前記ガイドシールドの前記上端部のスロットを含むことを特徴とする請求項22に記載の心臓弁システム。
- 先に存在する生体の弁輪内の自然心臓弁又は人工心臓弁と置換するための弁プロテーゼを受容するプロテーゼであって、
前記生体の弁輪内に移植可能な環状部材と、
前記環状部材から外側に伸びる縫合カフと、
前記環状部材及び前記縫合カフのいずれか一方から、上方及び/又は外側に伸びている複数のガイドシールドと、を含むことを特徴とするプロテーゼ。 - 前記ガイドシールドは、前記環状部材及び前記縫合カフのいずれか一方に、取り外し可能に取り付けられていることを特徴とする請求項24に記載のプロテーゼ。
- 前記ガイドシールドは、1つ以上の縫合糸によって前記縫合カフに取り外し可能に取り付けられていることを特徴とする請求項25に記載のプロテーゼ。
- 前記1つ以上の縫合糸の各々を1回切断すると前記ガイドシールドの各々が前記縫合カフから取り外し可能になるように、前記1つ以上の縫合糸が前記ガイドシールド及び前記縫合カフの穴を通って配列されていることを特徴とする請求項26に記載のプロテーゼ。
- 前記縫合カフは、前記縫合糸が切断されたときに前記縫合糸がアンカーループを通ってスライド可能になるように、前記縫合糸をそこに通して受容するための1つ以上のアンカーループを含んでいる
ことを特徴とする請求項26に記載のプロテーゼ。 - 前記環状部材及び前記縫合カフのいずれか一方に取り付けられた一端と、
前記弁プロテーゼを前記縫合カフに向かってガイドするために、前記縫合カフを通って受容可能である自由端と、
を含む複数の細長いリーダーをさらに含むことを特徴とする請求項24に記載のプロテーゼ。 - 前記ガイドシールドは、前記リーダーを解放可能に保持して前記環状部材の中心軸から離れた状態にするための1つ以上の要素を含むことを特徴とする請求項29に記載のプロテーゼ。
- 前記ガイドシールドは、前記ガイドシールドの間において前記弁プロテーゼを前記縫合カフに向けてガイドするための通路を少なくとも部分的に規定する内面を有していることを特徴とする請求項24に記載のプロテーゼ。
