Abstract
Description
The invention relates to an implantable connecting plate ge according to the preamble of claim 1 for attaching a elastic flat band on a bone.
Such a connecting plate is already from GB 20 39 220 A. known. There, a prosthetic ligament is described, in which at one end each of the ends of an elastic flat band expansion plate of the type mentioned is present. Each of these verbs has a slot through which the flat ribbon is carried out. The ends of the ribbon are with the Flat band firmly connected even in the central band area. The Ribbon can thus no longer ge from the connecting plates be solved.
The connecting plates each form a first part of a ver binding device, the respective second part firmly with a Bone is connected. The named parts of a respective verb Dungseinrichtung are releasably connected to each other, so that the part of the flat belt and connecting plates Easily detach and replace the prosthetic ligament.
The known ligament prosthesis, however, has two parts gene formation of the connecting devices a high number on components, so that they are relatively expensive and only take a long time effort is to be attached.
From EP 03 45 938 A1 it is also known an elastic pro to fasten the thesis band to a bone using clips which are hammered into the bones. Doing so the end of the prosthesis band after passing through the first clam always placed back on the prosthesis band over the clamp. With Hil A second parenthesis then becomes this covered end and the prosthesis band itself again fixed to the bone.
This type of attachment is relatively unreliable because the prosthesis band can slide out of the brackets over time, as has been described there in connection with Fig. 2c. In addition, the prosthesis band cannot be easily removed from the bone, since the clamps must first be pulled out of the bone for this. The prosthesis band can therefore not be easily removed during a reoperation.
The invention is based, the implantable Ver binding plate of the type mentioned so that a fastening of the elastic flat band on the bone below Use of a smaller number of components is possible. To addition should be achieved that the elastic ribbon in If necessary, can be easily removed from the connecting plate without that there is a risk that it will be unsafe during its use want to detach from the connecting plate.
The solution to the problem is in the characterizing part of Claim 1 specified. Advantageous embodiments of the Invention can be found in the subclaims.
An implantable connector plate according to the invention net is characterized by the fact that they little in addition to the first slot at least one second parallel to and opposite to it Slot contains, through which slots the ribbon passes is feasible that its draft-free end between the connection plate te and a tensile part of the ribbon comes to rest and runs in the direction of the tensile load.
The two slots in the connecting plate make it possible to the tension-free band end of the flat band under the below Arrange tensile force band, so that ultimately the span seamless end of the belt between the belt under tension and the connecting plate is clamped. This allows a very secure connection when the belt is under tension between the elastic ribbon and the connecting plate he aim. The connecting plate itself becomes suitable in a suitable manner attached to the bone.
In the event of a reoperation, the ribbon from the connection plate is loosened, so the flat under tension needs only slightly lifted from the associated flat ribbon end to who to be able to pull the ribbon end out of the slots nen. The connecting plate itself does not need the bone to be completely solved; it is sufficient to slightly tighten the fixing screw to solve. The flat strip end can be attached in an equally simple manner thread the already fastened connecting plate back into the slots, to re-connect the ribbon and the Manufacture connecting plate.
According to an advantageous embodiment of the invention, the Width of the slots within the connecting plate the width of the Flat ribbon. This ensures that the train tension flat ribbon and the tension-free flat ban end up lying on top of each other without lateral offset, what to an improved clamping of the tension-free bands of the tension-loaded flat strip and connection plate leads.
According to another advantageous embodiment of the invention ent speaks the height of the slots within the connecting plate of the Flat ribbon thickness. That passed through the slots Ribbon thus has very small bending radii in this area on, which causes the pinch of the ribbon within the Lets increase the connecting plate.
According to a further advantageous embodiment of the invention the implantable connector plate has a third slot between the first edge of the plate and the next one two first-mentioned slots and runs parallel to them, wherein the third slot has a height that is twice the thickness of the flat band.
The end of the flat ribbon can first through the third slot be threaded through to be next by the next one th slot and then through the one further away last slot to be passed. That through this last slit protruding ribbon end is then again through the third th slot passed so that it between the ribbon and the connecting plate comes to rest. The direction of pull is generally in the plane of the connecting plate, so now like around the flat ban passed through the third slot end by the flat band under tension against the Connecting plate is pressed. Because the flat ribbon overall passed through three slits results in an even firmer one Connection between it and the connection plate.
According to yet another advantageous embodiment of the invention the connecting plate has at least one through opening to accommodate a fixing screw, the slots in the loading rich between this through hole and the said he most plate edge come to rest. With the help of the fixing screw the connection plate can be fixed to a bone, whereby as a fixation screw, for example, a cortex screw on sentence can come. It can be so long that it is also in the opposite cortical bone can be screwed in.
According to a development of the invention, the through opening provided with a bevelled peripheral edge, whereby achieved is that the fixing screw can be partially countersunk. Here by the height of the connecting plate and fixation screw existing facility can be reduced.
According to another development of the invention, the verbin in the area of the through opening one to the rear have increased thickness. So there will be some freedom for the parts of the flat running below the connecting plate bands, which prevents the connection plate too large a tilted position after attaching it to the bone occupies.
According to a very advantageous development of the invention, the Connection plate on a opposite the first plate edge a second adjustment plate opening, in who, for example, coupled or hooked a spring balance that can. This can already be done with the connecting plate connected flat ribbon verse with a desired biasing force hen, which generally at the end of a tape operation the artificial tape is applied. The adjustment Through opening can also be designed as a slot that is parallel runs to the first-mentioned slots and opposite them lies. In this case, another ribbon can be used for transfer use the preload, which creates a certain richness tion stability is achieved when applying the bias, because the leader ribbon compared to a simple hook the La stabilized the connecting plate.
The connecting plate itself can be made of plastic, for example or made of metal. A titanium alloy is preferred used. The connecting plate can be used for another advantageous embodiment of the invention at least in the a rough or have structured surface in order to enlarge Increased friction between the connecting plate and the ribbon achieve. The connection between flat ribbon and connection This makes plate even more secure.
As elastic or artificial flat strips can in particular Cruciate ligaments, augmentation ligaments and support ligaments be viewed, for example, from TREVIRA-HOCHFEST stand. As a result of the slots mentioned, these bands can be fixed connect to the connecting plate without the risk of that the individual threads of the band construction are injured, which in general then the case is when these tapes are made using Screws or clamps are held. Through the verb dungsplatte according to the invention is the strength of the implan band in an unforeseen manner due to injury diminished by single threads. The result is a very safe one Anchoring a band, especially when it is Band is relatively narrow (<10 mm).
With a certain loss of bone strength (osteoporosis) it may be necessary to connect the respective connection plate with more to be attached to the bone as just a fixing screw. In this Case, the connecting plate can be several through have passage openings.
Es zeigen:The invention is described below with reference to the drawing described in more detail. Show it:

According to the Fig. 1 a connecting plate after OF INVENTION dung is provided with the reference numeral 1. The connecting plate 1 has a first slot 2 and a second slot 3 in its lower region. Both slots 2 and 3 face each other and run parallel to each other. They are symmetrical or perpendicular to a center line 4 running in the longitudinal direction of the connecting plate 1 (line of symmetry). In the middle of the line of symmetry 4 and above the first slot 2 there is a through opening 5 which serves to receive a fixing screw (not shown). A peripheral edge region 6 of the through opening 5 is chamfered on the observer's front side of the connecting plate 1 in order to be able to countersink a head of the fixing screw. A below the second slot 3 Randbe rich of the connecting plate 1 is referred to as the first plate edge 7 , which runs parallel to the second slot 3 . Opposite it is a second plate edge 8 , which is semicircular and extends concentrically to the through opening 5 or to the peripheral edge region 6 .
A longitudinal section along the center line 4 of the connecting plate according to FIG. 1 is shown in FIG. 2. On the basis of this longitudinal section it can be seen that a circumferential bead 9 is present on the rear side of the connecting plate 1 in the area of the through opening 5 . With this peripheral bead 9 , the connecting plate 1 comes to rest on a bone, not shown, on which it is to be fixed. The remaining rear region of the connec tion plate 1 then has a certain distance from the bone, so that a certain space for receiving an elastic flat band is obtained.
The flat strip is provided with the reference number 10 in FIG. 2. Since the tensile load direction is marked with an arrow 11 net. The direction of the tensile load is thus the direction in which the tensile force acts when the flat band and connecting plate are implanted in the body.
As FIG. 2 shows an end of the flat strip punched out 10 initially from the rear side of the connecting plate 1 fro through the first slit 2, and then eingefä in the second slot 3 12, so that the end 12 is again that of front to back Connection plate 1 arrives. There the end 12 is surrounded so that it extends in the direction of tensile load 11 . The end 12 of the flat strip 10 thus comes to lie under the part 13 of the flat strip which is subjected to tensile stress, wherein it is pressed by this part 13 which is subjected to tensile stress against the connecting plate 1 . It is practically between the loaded part 13 and the connecting plate 1 a clamped. The connecting plate 1 , the end 12 of the ribbon 10 and the loaded part 13 of the ribbon 10 run parallel to each other.
The width of the first and second slot 2 and 3 corresponds to the width of the flat band 10 , while the thickness of the flat band 10 corresponds to the height of the respective slot 2 and 3 . The width of the flat strip 10 can be, for example, in the range from 3 to 15 mm, and its thickness can be 0.8 mm. The width and height of the slots 2 and 3 are selected accordingly.
A connection plate 1 shown in Fig. 3 'is similar to the Ver connection plate 1 , but has a third slot 14 which is between the first plate edge 7 and the second slot 3 . The third slot 14 has twice the height of the first and second slots 2 and 3 . The reason for this is that the elastic flat strip 10 is passed through the third slot 14 twice. All slots 2 , 3 and 14 can be arranged at the same distance from each other and are pa rallel to each other. The third slot 14 is also symmetrical to the center line 4 and the slots 2 and 3 opposite.
In Fig. 4, which shows a longitudinal section along the center line 4 of Fig. 3, a fixing screw 15 is shown, which has a screw head 16 . This fixing screw 15 extends through the through opening 5 from the front to the rear of the connecting plate 1 'and can be, for example, a cortex screw. Your screw head 16 nestles partially on the tapered peripheral edge region 6 and is consequently partially sunk inside half of the connecting plate 1 '. The screw head 16 can have a polygonal recess on its end face, into which an Allen key can be inserted appropriately.
A solid connection between the elastic ribbon 10 and the connecting plate 1 'to manufacture, the end 12 of the flat strip 10 is initially manure plate from the back of Verbin 1' out through the third slot 14 through to the front of the side and then second by the Slot 3 through again to the back of the connecting plate 1 '. Then ge reaches the end 12 through the first slot 2 through again to the front of the connecting plate 1 'and then across the second slot 3 and through the third slot 14 through again to the rear of the connecting plate 1 '. After the end 12 has passed through the third slot 14 for the last time, it is bent in the direction of the load 11 so that it comes to rest below the part 13 of the flat belt 10 which is subject to tension. The flat ribbon 10 is thus passed twice through the third slot 14 , so that it must have twice the height of the slots 2 and 3 . The connecting plate 1 ', the end 12 of the ribbon 10 and the tensile portion 13 of the ribbon 10 run again parallel to each other rum, so that the free end 12 is pressed under tension by the part 13 against the connecting plate 1 '. It is again clamped here between the part 13 and the connec tion plate 1 '. In contrast to the exemplary embodiment according to FIG. 2, the band end 12 is clamped here not only at the rear of the connecting plate 1 ', but also at its front in the area between the slots 3 and 14 . This leads to an even firmer connection between ribbon 10 and connecting plate 1 '.
Another embodiment of a connecting plate is shown in FIG. 5. The connecting plate here bears the reference numeral 1 '' and differs from the connecting plate 1 'in that above the through opening 5, an adjustment slot 17 is present. This adjustment slot 17 is thus in the upper region of the connecting plate 1 '' or above the through opening 5 , while the other slots 2 , 3 and 14 in the lower region of the connecting plate 1 '' or below the through opening 5 are po sitioned. The adjustment slot 17 is arranged symmetrically or right angled to the center line 4 , runs parallel to the slots 2 , 3 and 14 and lies opposite them. It serves to accommodate an adjusting and tensioning band for setting a desired tension for the elastic flat band 10th In this case, the second plate edge bears the reference symbol 8 a and runs parallel to the adjustment slot 17 in the central region. The entire connecting plate 1 '' has a practically rectangular shape in the present case, the corners being rounded.
Fig. 6 shows a longitudinal section along the center line 4 in Fig. 5. Through the adjustment slot 17 , an adjustment and tensioning strap 18 runs through, which can be pulled in the direction of arrow 19 and thereby takes the connecting plate 1 ''. Is the elastic flat band 10 with the connecting plate 1 '' firmly connected, it can be biased in the desired manner when moving the connecting plate 1 '' in the direction of arrow 19 due to the pulling action of the adjusting band 18 . The bias can be adjusted, for example, with a spring balance, which is coupled to the adjusting band 18 . After adjusting the bias of the ribbon 10 , the fixing screw 15 is screwed into the bone, not shown, to attach the connecting plate 1 '' to the bone.
In a modification of the examples shown in FIGS . 1 to 6, the through opening 5 can also be designed as an elongated hole which extends in the direction of the center line 4 . In this case, a bias of the flat belt 10 can be adjusted with the help of the adjusting belt 18 even when the fastening point is fixed relative to the bone and the connecting plate 1 '' must still be moved.
