AIM: To identify the risk factors for aneurysmal intra-procedural rupture (IPR) of intracranial a... more AIM: To identify the risk factors for aneurysmal intra-procedural rupture (IPR) of intracranial aneurysms suring endovascular treatment, and to discuss its management and outcomes. MATERIAL and METHODS: A total of 106 patients with cerebral aneurysm who have undergone endovascular treatment (EVT) were included in this study, with a mean follow-up period of 17.2 months. Risk factors for IPR, such as the aneurysm's location, size and morphology, etc. were evaluated, and the chi-squared test was used for statistical analysis. Clinical outcomes were assessed using the modified Rankin scale (mRS) at 15 months. RESULTS: Among all 106 patients who have undergone EVT, five (4.7%) had aneurysmal IPR as well as ruptured aneurysms with subarachnoid haemorrhage (SAH). Among those five patients, primary coiling was performed in three cases and balloon/stentassisted coiling was performed in two cases, with complete occlusion of the aneurysmal sac achieved in four (4/5) patients. Clinical follow-up with the mRS scores of the patients revealed mortality in one patient (20%), favourable outcomes in three patients (60%) and unfavourable outcomes in one patient (20%). CONCLUSION: Aneurysmal IPR is considered a rare but important complication of endovascular coiling that is associated with poor clinical outcomes. Several risk factors are significantly associated with IPR, such as small-sized aneurysm (<3.5 mm), presence of bleb and parent vessel tortuosity. Acom location, irregular shape and past episode of SAH also increase the risk of IPR, but such association was found to be statistically insignificant in our study. Although IPR is considered a devastating complication, good clinical outcomes can be achieved with early detection and proper management using rapid aneurysmal coil packing and occlusion.

Abstract Craniopharyngiomas (CPs) are histological benign lesions that can develop at different p... more Abstract Craniopharyngiomas (CPs) are histological benign lesions that can develop at different positions along the pituitary–hypothalamic axis from the sella to the third ventricle. A common characteristic of pediatric CPs is the presence of a large suprasellar cystic component and symptoms are mainly caused by direct compression over adjacent neurovascular structures and obstruction of the Cerebrospinal Fluid (CSF) flow pathway. They represent a distinctive management challenge, and aggressive surgical resection can have major postoperative complications and adverse outcomes. The transcranial endoscopic decompression of cysts followed by the placement of an Ommaya reservoir is a safe, minimally invasive technique used to abolish symptoms caused by the pressure effect. In this study, we present our experience of managing cystic CPs of pediatric age using the transcranial endoscopic approach. Six consecutive cystic CP cases operated on using the transfrontal transventricular approach between 2015 and 2018 were retrospectively reviewed. An endoscope was introduced through a Kocher’s burr hole, the cyst fenestrated, and the contents gently aspirated using an angiography catheter and a 5-ml syringe. The cyst wall, free from surrounding structures, was excised, and the remnants coagulated. An Ommaya reservoir was placed in all cases. Headache and hydrocephalus were relieved in all cases (6/6), and visual improvement was observed in 5/6 cases. Post-operative diabetes insipidus and hypothalamic dysfunction were not observed in any patient. One patient with cyst recurrence at follow-up underwent subtotal resection followed by adjuvant radiotherapy. Our experience with limited number of cases demonstrates that the transcranial endoscopic approach is a minimally invasive and safe technique for the decompression of cystic CPs. It provides acceptable long-term disease control and circumvents the complications associated with radical surgery.

Studies on the hepatoprotective activity on methanolic extract and sub fractions of leaves and the chloroform extract and sub fractions of roots of Eclipta alba was carried out using carbon tetrachloride- induced liver damage and Lysosomal enzymes level in wistar albino rats. The methanolic extract of leaves and the chloroform extract of roots of Eclipta alba showed significant activities (P<0.01) and (P<0.05) respectively causing 72.8% & 47.96% reduction of lysosomal enzyme. The triterpenoid Eclabasaponin fraction from methanolic extract of leaves produced significant (P<0.001) (78.78%) and the alkaloidal fraction (P<0.05) (60.65%) reduction of carbon tetra chloride induced increase in lysosomal enzyme in blood. The Coumestan fraction (CF) and Triterpenoidal saponin fraction (RTF) from the chloroform extract of roots produced very significant (P<0.01) (75.6%) and (P<0.05) (52.41%) respectively reduction of carbon tetra chloride induced increase in lysosomal enzyme levels in blood.
BACKGROUND Stereotactic brain biopsy for eloquent area brain tumours is associated with complicat... more BACKGROUND Stereotactic brain biopsy for eloquent area brain tumours is associated with complications like hemorrhage due to multiple sampling and increased operative time due to dependence on neuropathologist for frozen section assessment. In order to curb these limitations, we investigated the use of Fluorescein sodium fluorescence in confirming pathological tissue samples in stereotactic brain biopsy of gadolinium-enhancing tumours. METHODS This prospective observational study included 23 consecutive patients who underwent stereotactic biopsy of gadolinium-contrast-enhancing brain lesions with intravenous fluorescein sodium administration. 93 specimens were obtained and examined for the presence of fluorescence using a microscope with fluorescence visualisation capability. Later the nature of the samples was confirmed on histopathology. The sensitivity and specificity of flourescein sodium fluorescence were calculated based on histopathological confirmation. RESULTS 58 (62.3%) fluorescent and 35 (37.6%) nonfluorescent samples were obtained. All fluorescent specimens contained diagnostic tissue appropriate for tumor grading. Of 35 nonfluorescent specimens, 12 (34.3%) did not contain tumor, 11 (31.4%) contained minor hypercellularity or gliosis, and 12 (34.3%) contained tumor with a high proportion of necrosis. The sensitivity and specificity for fluorescein fluorescence were 83% and 100%, respectively. CONCLUSIONS Fluorescein fluorescence is a handy tool to detect the pathological tissue in stereotactic brain biopsy and may improve its diagnostic accuracy and expedite the procedure.

Many techniques are available to treat AAD but there are some specific conditions where some techniques have advantage over the other. Technical Advantage: C2-3 transfacetal screw with standard C1 lateral mass fixation provides a stronger construct because of four cortices incorporation and divergent course of screws and is biomechanically comparable to other forms of C2 fixation techniques. It is a technically less demanding and time-consuming. It is also a good alternative in cases having significant osteopenia. Conclusion: C2-3 transfacetal screw with standard C1 lateral mass fixation is an effective alternative to routine C1 lateral mass and C2 pedicle/pars screw fixation in cases of atlantoaxial dislocation complicated with high riding or posteriorly placed vertebral artery and thin pedicle of C2 vertebra.

Background: Low back pain is a common medical problem in general population with high morbidity a... more Background: Low back pain is a common medical problem in general population with high morbidity and healthcare costs. The optimal management strategy, including the role of surgical intervention, remains controversial. We conducted a randomized prospective cohort study to compare the efficacy of surgery and conservative management for chronic low back pain. Materials and Methods: This randomized prospective cohort study was conducted on 130 patients with low back pain. Patients were divided randomly into Group I (Conservative management) and Group II (surgical management). Follow up of patients was done at intervals of 1 week, 4 weeks, 3 months and then 6 month, 1 year and 2 years. Visual Analog Scale (VAS) was used for assessment of pain for all patients. Modified Mc Nab Scale was used as post operative/management tool to assess intensity of low back pain and quality of life. Results: There was a gradual decline in VAS score for Group 1 till follow up whereas an early and sharp decrease in VAS scorewas seen for Group II till 3 months after which it increased on an average till last follow up. Mac Nab scale showed gradual improvement in condition of patients over a period of time for Group I with more patients showing improvement by 1 year. Mc Nab scale showed good results starting within first week only up to 3 months for Group II. Later there was some deterioration/static scores in group 2 perticularly, and patients were almost in same state at 2 years of follow up for both the groups. Conclusion: A gradual improvement in condition occurs by conservative management for chronic low back pain. Surgical management provides immediate good results but in some patients pain relief may not last for long period. More studies comparing the two treatment options can give better insight into the selection of treatment option for the patients with chronic low back pain and may validate our results.

The aim of this pictorial essay is to show a wide spectrum of representative cases, which are frequently misdiagnosed as PIRADS ⅘ while reporting mp-MRI of the prostate. We provide some valuable recommendations to avoid these fallacies and improve mp-MRI of prostate evaluation.

Nudges were delivered via text message to over 700,000 Walmart pharmacy patients in the fall of 2020 and encouraged patients to visit Walmart for a flu vaccine. We demonstrate that the best behaviorally informed messages can increase pharmacy vaccination rates by up to 8.3% compared to the least effective messages over a roughly one month follow-up period. The most effective messages in our field experiment matched the tone of typical pharmacy communications and reminded patients that a flu shot was waiting for them. These insights suggest that carefully crafted messages informed by the results of this study could nudge the adoption of other vaccines for other infectious diseases, including COVID-19.

Typhoid ileal perforation lead to secondary peritonitis that is a life threatening condition. Assessing the severity of peritonitis is important step in determining the proper treatment plan. AIM: This study is aimed to assess the ability of modified APACHE II (Acute Physiology and Chronic Health Evaluation II) score to correctly assess the severity of generalized peritonitis secondary to ileal perforation. MATERIALS AND METHODS: 100 patients of peritonitis secondary to ileal perforation were chosen in a one year period and were applied modified APACHE II score to assess the severity and outcome. RESULTS: Most of perforation occurred in 2nd and 3rd decade (65%). APACHE II score ranged from 1 to 21. Most of patients had < 9 score. Higher APACHE II score was associated with increased mortality. Morbidity was also accurately predicted by APACHE II score. CONCLUSION: Typhoid ileal perforation is still a common health problem causing high mortality (11%). Compared to mortality, morbidity was less accurately predicted by APACHE II score, and more number of the patients developed more complication in higher APACHE II score group.

Murraya koenigii L. (family: Rutaceae) commonly called as curry leaf is a highly valued plant for... more Murraya koenigii L. (family: Rutaceae) commonly called as curry leaf is a highly valued plant for aroma and medicinal value. The two dominant AM species Glomus mosseae and Acaulospora laevis were isolated from the rhizospheric soil of M. koenigii. A pot experiment was performed to see the interactive potential of G. mosseae and A. laevis alone or in combination with Pseudomonas fluorescens on M. koenigii. Various morphological and biochemical parameters were measured after 120 days. Overall results suggest that although, all co-inoculation treatments showed beneficial effects on all the growth, physiological and oil content. The overall results demonstrate that the co-inoculation of bioinoculants like P. fluorescens with AM fungi promotes higher AM colonization and spore number enhancing nutrient acquisition especially phosphorus (P), improving the rhizospheric condition of soil.

CONCLUSIONS Fluorescein fluorescence is a handy tool to detect the pathological tissue in stereotactic brain biopsy and may improve its diagnostic accuracy and expedite the procedure.

Many techniques are available to treat AAD but there are some specific conditions where some techniques have advantage over the other. Technical Advantage: C2-3 transfacetal screw with standard C1 lateral mass fixation provides a stronger construct because of four cortices incorporation and divergent course of screws and is biomechanically comparable to other forms of C2 fixation techniques. It is a technically less demanding and time-consuming. It is also a good alternative in cases having significant osteopenia. Conclusion: C2-3 transfacetal screw with standard C1 lateral mass fixation is an effective alternative to routine C1 lateral mass and C2 pedicle/pars screw fixation in cases of atlantoaxial dislocation complicated with high riding or posteriorly placed vertebral artery and thin pedicle of C2 vertebra.

In this sheet layer of glass fibre in woven form & layer of thin bamboo fibre is used. As per ASTM standard, specimen are prepared & followed by flexural test, tensile test & hardness test. It is observed that bamboo/glass fibre laminates have influenced mechanical properties.

Background: Low back pain is a common medical problem in general population with high morbidity a... more Background: Low back pain is a common medical problem in general population with high morbidity and healthcare costs. The optimal management strategy, including the role of surgical intervention, remains controversial. We conducted a randomized prospective cohort study to compare the efficacy of surgery and conservative management for chronic low back pain. Materials and Methods: This randomized prospective cohort study was conducted on 130 patients with low back pain. Patients were divided randomly into Group I (Conservative management) and Group II (surgical management). Follow up of patients was done at intervals of 1 week, 4 weeks, 3 months and then 6 month, 1 year and 2 years. Visual Analog Scale (VAS) was used for assessment of pain for all patients. Modified Mc Nab Scale was used as post operative/management tool to assess intensity of low back pain and quality of life. Results: There was a gradual decline in VAS score for Group 1 till follow up whereas an early and sharp decrease in VAS scorewas seen for Group II till 3 months after which it increased on an average till last follow up. Mac Nab scale showed gradual improvement in condition of patients over a period of time for Group I with more patients showing improvement by 1 year. Mc Nab scale showed good results starting within first week only up to 3 months for Group II. Later there was some deterioration/static scores in group 2 perticularly, and patients were almost in same state at 2 years of follow up for both the groups. Conclusion: A gradual improvement in condition occurs by conservative management for chronic low back pain. Surgical management provides immediate good results but in some patients pain relief may not last for long period. More studies comparing the two treatment options can give better insight into the selection of treatment option for the patients with chronic low back pain and may validate our results.

It is important not only to detect these suspicious lesions but also to be aware of the potential pitfalls in mp-MRI prostate imaging. The aim of this pictorial essay is to show a wide spectrum of representative cases, which are frequently misdiagnosed as PIRADS ⅘ while reporting mp-MRI of the prostate. We provide some valuable recommendations to avoid these fallacies and improve mp-MRI of prostate evaluation.

We partnered with Walmart to test 22 nudges designed to boost vaccination rates in their pharmaci... more We partnered with Walmart to test 22 nudges designed to boost vaccination rates in their pharmacies. Nudges were delivered via text message to over 700,000 Walmart pharmacy patients in the fall of 2020 and encouraged patients to visit Walmart for a flu vaccine. We demonstrate that the best behaviorally informed messages can increase pharmacy vaccination rates by up to 8.3% compared to the least effective messages over a roughly one month follow-up period. The most effective messages in our field experiment matched the tone of typical pharmacy communications and reminded patients that a flu shot was waiting for them. These insights suggest that carefully crafted messages informed by the results of this study could nudge the adoption of other vaccines for other infectious diseases, including COVID-19.

Typhoid ileal perforation lead to secondary peritonitis that is a life threatening condition. Assessing the severity of peritonitis is important step in determining the proper treatment plan. AIM: This study is aimed to assess the ability of modified APACHE II (Acute Physiology and Chronic Health Evaluation II) score to correctly assess the severity of generalized peritonitis secondary to ileal perforation. MATERIALS AND METHODS: 100 patients of peritonitis secondary to ileal perforation were chosen in a one year period and were applied modified APACHE II score to assess the severity and outcome. RESULTS: Most of perforation occurred in 2nd and 3rd decade (65%). APACHE II score ranged from 1 to 21. Most of patients had < 9 score. Higher APACHE II score was associated with increased mortality. Morbidity was also accurately predicted by APACHE II score. CONCLUSION: Typhoid ileal perforation is still a common health problem causing high mortality (11%). Compared to mortality, morbidity was less accurately predicted by APACHE II score, and more number of the patients developed more complication in higher APACHE II score group.

Murraya koenigii L. (family: Rutaceae) commonly called as curry leaf is a highly valued plant for... more Murraya koenigii L. (family: Rutaceae) commonly called as curry leaf is a highly valued plant for aroma and medicinal value. The two dominant AM species Glomus mosseae and Acaulospora laevis were isolated from the rhizospheric soil of M. koenigii. A pot experiment was performed to see the interactive potential of G. mosseae and A. laevis alone or in combination with Pseudomonas fluorescens on M. koenigii. Various morphological and biochemical parameters were measured after 120 days. Overall results suggest that although, all co-inoculation treatments showed beneficial effects on all the growth, physiological and oil content. The overall results demonstrate that the co-inoculation of bioinoculants like P. fluorescens with AM fungi promotes higher AM colonization and spore number enhancing nutrient acquisition especially phosphorus (P), improving the rhizospheric condition of soil.
