It was made up of polyethylene high density sheet according to the measurement of the individual’s thigh. Union occurred in all the cases and function was far advanced by the time clinical and radiological union occurred. Thigh lacer provides stabilizing influence at the fracture site allowing uninterrupted fracture healing and negligible movement at the fracture site. It gave gratifying results in the fractures of the middle and lower third of femur but not in the fractures of the upper third femur. Thigh lacer was not found detrimental to the fracture union.

Clinical circumstances can potentiate this process, such as acute infection or insulin discontinuation. Additionally, patients on SGLT2-inhibitors are at risk for euglycemic DKA. In people with type 2 diabetes, DKA is uncommon; however, a combination of precipitating factors in these patients can lead to a greater risk of DKA, particularly in the setting of SGLT2-inhibitor use. Clinical Case A 63 year old male with past medical history significant for uncontrolled type 2 diabetes (10 year duration, HgA1c=11.2%, on insulins detemir and aspart, metformin, and empagliflozin), coronary artery disease, and treatment refractory antibody-negative polymyositis (baseline CPK levels ~1000-2000, on a burst of prednisone for flare) presented with feve...
Wound healing trajectories (percent healing vs time) provide a dynamic picture of the decrease in... more Wound healing trajectories (percent healing vs time) provide a dynamic picture of the decrease in wound burden over the entire continuum of the healing process. Trajectories can be robustly compared using survival statistics methodology. Improvement in healing can be determined by shifting the curve from "impaired" healing toward "ideal" healing. Although this concept of shifting the curve "to the left" has been demonstrated in acute incisional healing depicted by the gain in tensile strength, and in other chronic wounds, it has not been utilized for chronic pressure ulcers. Wound healing trajectories were constructed for 211 patients enrolled in 8 separate randomized clinical trials for grade III and IV pressure ulcers. Trajectories were constructed for patients achieving ≥90% or more healing within 112 days and those who achieved less than <90% wound closure.
