The Pierre Robin sequence (PRS) is characterized by the triad of micrognathia, glossoptosis, and ... more The Pierre Robin sequence (PRS) is characterized by the triad of micrognathia, glossoptosis, and airway obstruction. Conservative management in Pierre Robin sequence consists of nasogastric tube feeding and positioning of the neonate (prone or lateral position) that facilitates the anterior position of the tongue or the application of continuous positive nasal pressure. In case of failure of this treatment, emergency tracheostomy and/or mandibular distraction must be performed. Mandibular distraction is a standard technique used by craniofacial surgeons to achieve an anteroposterior horizontal lengthening of the mandibular body, correcting the posterior position of the base of the tongue and thus retropharyngeal enlargement of the airway. The authors present 2 clinical cases of hypertrophy of the sublingual salivary glands with the use of mandibular distractors in SPR patients with severe airway obstruction.

Due to the vast phenotypic variation and complexity of the malformation, a global treatment that is coordinated by a multidisciplinary team is imperative. Herein, the authors describe 3 clinical patients and discuss the different therapies used according to the type of microsomia present. It was concluded that early and integrated treatment, which considers all the affected and potentially affected soft and hard tissue, is needed, and that patients must be monitored until they have finished growing.

Este estudio evalúa la eficacia de la distracción mandibular como tratamiento precoz en PRS con dificultad respiratoria severa. Pacientes y Método: Lactantes SPR controlados en Unidad Cirugía Plástica Hospital Luis Calvo Mackenna entre enero 1998 y marzo 2003. La indicación de distracción se basa en evaluación clínica, radiología, nasofibroscopía, saturación de oxígeno y/o polisomnografía. El seguimiento tardío evalúa complicaciones, evolución nutricional, crecimiento y desarrollo facial. Resultados: 23/31 lactantes SPR se les realizó distracción mandibular, presentando 100% alivio de sintomatología obstructiva. Dos casos presentaban traqueotomía desde RN, que se retiraron durante el procedimiento. Dos casos presentaban hipertensión pulmonar que revirtieron post distracción. Hubo mejoría significativa de las curvas de crecimiento ponderal. Conclusión: La distracción mandibular es un método efectivo en lactantes con micrognatia, aliviando la obstrucción respiratoria, facilitando alimentación, evitando raqueotomías y/o permitiendo decanulación precoz. Distraction osteogenesis has been used to increase maxillary advancement. Methods: Six patients with severe maxillary hypoplasia, CL and VPC were selected. They underwent distraction osteogenesis using a distraction device type RED II, intraoral orthodontic anchorage device and underwent Le Fort I osteotomy. Cephalometric analyses using lateral radiographs were performed pre and postdistraccion. Results: The SNA, ANB and convexity angle increased post distraction and were considered statistically significant. The patients had a positive correction of its horizontal overjet of 10.2 mm. Conclusion: The use of rigid external distraction can reposition a retruded maxillary to the position desired in a very stable way.

Eligibility criteria: We will include randomized trials evaluating the effect of autologous platelet derivatives on newly bone formed after secondary alveoloplasty in cleft lip and palate patients. Two reviewers will independently screen each study for eligibility, data extraction, and bias assessment using the Cochrane "risk of bias" tool. We will pool the results using meta-analysis and apply the GRADE system to assess the certainty of the evidence for each outcome. Data sources: A comprehensive search will include all relevant randomized controlled trials (RCTs), the ongoing investigation reported in specialty congresses and trials regardless of language or publication status (published, unpublished, in press, and progress).
