Note that this will run when the lifespan is triggered most of the time but if the lifespan is disable it will run on the first request. So not checking scope["type"] is by design.
I don't think I have lifespan disabled, yet I'm not seeing the middleware initialized until the first HTTP request is received. How do I force eager initialization of the middleware?
